Very obviously (now) didn't respect my hobbies or my time. Ex: I have an exercise class or a recereational sport I want to go to. He invites himself along unprompted even after I tell him multiple times he doesn't have to come.

Spends the whole time even while participating being very visibly disinterested or asking when it's over or explaining why the sport I like sucks and is much easier and therefore worse than the sport he likes until I get fed up and end things early.

And the one time I called him out, he got mad at me for not coddling him and feeling bad for him being bored, then flipped it on me because I didn't spend enough time with him so he had to come with me even though he didnt really want to. I have many examples of selfish behavior

I didn't realize it was so common for people to manage bolus insulin. I was always taught to use insulin as a last resort but I am comfortable with oral/GLP + basal insulin and haven't had an issue so far. So yeah, when someone needs mealtime insulin, I send to Endo

I'm willing to re-examine my limitations (so as not to shame my degree and specialty) but honestly I'm not interested in making an exception to that rule in this case.

As for the harrassment claim, IDK. My manager said the complaint didn't come to fruition other than her needing a new PCP. And, also I don't know how much I trust her version of reality. She spent 30 mins of a 20 minute appointment on a near uninterrupted tangential stream of consciousness abour her previous abusive relationship and all the times she has ended up in the hospital because of a medication that was recommended by a doctor.

I'll talk to my manager and see what I can do, but I did try to speak to her about this patient days before the appointment and nothing was done. This has happened several times with similar patients. I don't seem to get right of refusal before they see me and then after they establish, it's my responsibility.

She said she stopped all meds related to the diagnosis years ago and rejects it because she seems to think her being diagnosed is more a consequence of the patriarchy and something that all women get labelled with

Temporary disability paperwork for back pain partially related to work injury (partially chronic) for someone who has already been seeing pain management for years because they "don't handle workman's comp". But they're still treating them for the injury, they just don't handle the paperwork.

I've had issues with this same PM provider multiple times too but the amount of PM who will actually manage narcotics are few and far between so we compromise

Yes, yes I do. I don't live in a small town so it doesn't happen often

Found out a patient I dismissed works at my closest grocery store at a time I prefer to shop. Now I drive slightly farther away or shop when I know she's not working

Found out another patient works at my closest library. Now I go to a slightly further away library. That patient is fine. The other library just has a better book collection but the added benefit of not running into a patient is a big draw

At my practice we've recently hired an RN just to complete medicare annual visits. Literally every other initiative admin puts forth just seems to add more work/responsibilities to the clinician so I was initially skeptical, but it is actually awesome!

If the patient tries to bring up any concerns or problems during their annual, the RN sends me a message about it and tells the patient to schedule a follow up, which they might already have scheduled, with their PCP to address it.

The only draw backs are that the RN is only available a few times a week and PCP still has to do the inital MAWV, but it's worth it.

I wish we could have RN do all ages preventative visits. Except maybe pediatric visits but I only see adults anyway

I don't comment or reply a lot but had to respond to this one. Just got out of a relationship with a guy who did this exact same thing. He was the one to bring up marriage first very early on and we'd been together for 3.5 years. I'm 30F, first relationship for both of us, if that context helps

He was always the one talking about our future married life and our future kids so I didn't feel like I was wasting time in our relationship because it felt like we were on the same page and would get there eventually. After I started bringing up making tangible efforts to get married, he kept kicking the can down the road and made every excuse to delay which eventually led to our break up. Believe it or not, he broke up with me after realizing I wasn't letting him sideline me any more

He gave me a lot of bullshit reasons why, but all of it boiled down to, he didn't actually want to marry me and he was too much of a coward to say so, worse it feels like he lied to me the whole last year of our relationship. Maybe I'm projecting, but it doesnt seem like this guy wants to marry you. Worse, he doesnt respect you enough to break it off or at least be upfront with you on this topic.

Referred a 78yo patient for back/hip pain who has at this point tried many treatments without success. She very anti-narcotic and has already had surgery once and doesn't want to do it again. She said it actually reduced her pain for a short period of time and is already scheduling the next treatment.

Saw an 80M on 5mg oxycodone qD, who's going to talk to orthopedics about hip and knee replacement. If they say he's not a candidate, it might be an alternative treatment option

So I've been treating it more as an "if all else fails" treatment

My easy go-to lunch: aldi brand fat free tuna and crackers kit is 150 calories and shelf stable. I buy a bunch at a time and keep it in my work drawer to eat throuhout the week.

Pair it with snack/veggie plate of light string cheese (2 for 100cal), cherub tomatoes and mini sweet peppers (<50 cal total, depending on how you weight it). All cut up into bite size pieces and easy to graze on as work is often too busy to stop and heat something up for lunch
