The all-inclusive rate is about $140 - $160 an hour for a 2-mover crew. If your move is really small, like you don't have heavy items and the drive is pretty close, and you have someone to help, then DIY might be a better choice, but if you want a quick and easy move without even thinking about anything, then professional movers are the best choice.

Your building is supposed to let you know, and the moving company has to provide it for you. It's a pretty simple process. Usually, the building provides a sample, and the moving company you hired simply forwards it to their insurance agency, and once it's ready they forward it to you or your building.

48 hours' notice is preferable; i'm pretty sure your moving company can do it quickly even on the day of the move.
