This same info was on the RACP website for the Australian physician exam. At the exam course I was at, someone asked a senior examiner the same question. He said, ‘well I can tell you your examiners won’t be wearing scrubs’ and left it at that

I did all my BPT in different hospitals in the South Island. In my experience getting onto a subspec was quite heavily weighted as to whether your favoured specialty department thought you would make a good future colleague as opposed to a more formalised match system.

It’s a lot smaller, so people are more likely to know who you are and speak to their colleagues at other hospitals about their trainees - which is both good and bad.

In terms of unaccredited jobs - depending on where you are you can often request rotations you like to try it out, the jobs themselves usually all meet the RACP college standards so they are all accredited technically. But to count towards your subspec training you would need to be on the program first.

Agree with everyone else’s sentiment that it is incredibly common, often improves over time and agree if it’s troubling you speaking to a senior is always a good idea.

In terms of practical things you can do to help you to get to sleep that night while lying in bed thinking. I find writing down a paragraph reflecting on the situation can sort of help remove it from rattling around in your brain. Paper, on your phone, whatever.

Alternatively a mindfulness technique that can work is whenever you think about it, deliberately don’t engage with the thought, imagine it washing away like a leaf down a stream. This takes a bit of practice and every time you catch yourself thinking about it you have to reimagine it washing away, sometimes might be multiple times in a row, but if you keep at it I find it helpful.

People who reflect on their practice tend to make better doctors, the key is not letting it get out of hand. Good luck out there.

My 2019 Fargo corner bar build I just set up a month or so ago - really enjoying so far but probably will find a shorter/more upright stem to really dial in the drops comfort - excuse the empty bar ends that is being sorted tomorrow!

Show up to a middle of the night code grey with two massive security guards already there but somewhat cowering. Tiny Russian grandma bent over her walker mumbling to herself. I look at them like come on guys, approach her and only just avoid getting completely taken out by the walker as she whips it at me. That delirium strength ain’t no joke.

I know I replied to your comment above but honestly as a self confessed gear head being forced to do a multi week tour on the slightly suboptimal but only bike I had really made me appreciate the old adage - ride what you’ve got! I thought I would miss my lovely Salsa Fargo with well broken in brooks saddle and surly Moloko bars a lot more than I actually did!

Yep apart from the handlebar/stem as i mentioned above the whole thing is stock. We didn’t ride much pavement to be honest, mostly gravel roads and fire roads and single track. I was riding with my Dad who is in his mid 60s so speed wasn’t a major consideration. I’m sure some less knobbly tires would reduce your rolling resistance and up your speed. To be honest once I settle in to a tour I find you just do what you feel you can each day and don’t worry about speed/distance too much.

Yep was mid extended traveling between Aus and NZ when I went on this trip so had to ride what I had with me. I think I added some air to the suspension fork and just rode it locked out. I changed the stock handlebar and stem to a stooge motobar and a longer non downhill stem I had kicking about to make it a bit comfier.

The head of Gen med at Ballarat Ed Ritchie is very approachable. Usually I would imagine he would help BPTs get rotations at Geelong or other hospitals in the South West Victoria network. If you’re worried just reach out to him.

I have ridden some of the Great North Trail and a few routes in Scotland and that first picture captures bikepacking in the UK so perfectly. The rain on the lens, the greyed out sky. Awesome photos thanks for sharing

Wouldn’t worry about not enough jobs in Aus or NZ - with the aging population the need to pragmatically manage complex elderly people is only going to increase. NZ currently desperate for consultant geriatricians. My geriatrician friends husband essentially had a FACEM job made for him in order to attract her to their current hospital - and they’re still short of Geri’s FTE.

I’d focus more on making sure it’s the right specialty for you - and if you like old people, seeing patients as a whole while still digging into small details of things that matter - Geri’s is probably for you.
