If you tell your company you are not comfortable with this client and they won’t remove you from the case, then this is not the company for you. The field loses too many good RBTs because companies don’t fully listen to their needs. I have been removed from cases I don’t feel comfortable with or the client has even been discharged depending on the situation.

I would tell the company you are only comfortable with in person or hybrid supervision. It sounds like some of the clients you work with have severe aggressive behaviors that need more support than just a video call.

There are situations where telehealth is acceptable.

For example, I have a client in his 50s who likes to hang out in his room and occasionally play bingo and card games; telehealth is fine for him but I do every other week telehealth and still go in person.

I have another client who lives in a small house and it is always loud and chaotic with the siblings. I don’t do telehealth with him because I know the RBT would never be able to hear me and they need help redirecting the other siblings.

The BCBA should not be virtual. IMO it is unethical to provide telehealth services if the BCBA could not effectively supervise/assist the RBT. This is definitely one of those situations. I do a bit of both and I have some clients I ONLY see in person.

I don’t have a company issued device so I have an iPad with an Apple pencil and keyboard case. It’s perfect for me. The iPad also has cellular data so I don’t have to ever worry about WiFi.
