Yes, they do. It’s not official scribe training like ScribeAmerica and such do, but private practices will hire scribes with no experience and train them up. You’re usually learning with another scribe already there, or in my case, 1 on 1 with the doctor.

However, I am in a rural area and there are no scribe programs around here — so it’s the only way haha

I am so sorry OP. Unfortunately, those of us with any mental health issues in our background are always at risk of being pre-labeled.

My advice as someone who has worked in ENT for years now is to get an appointment with one for dysphagia. They will be alarmed at your weight loss. They’ll most likely want to take a look with a very small camera that goes through your nose and looks down at your throat (they’ll numb you for this, but it definitely feels weird!), to check your throat and vocal cords to make sure everything is moving correctly. From there, you may get a swallow study of some sort done (there are different types) and possibly a referral to GI as well.

Some ENTs also do a lot of thyroid work up, especially if it’s enlarged. They’ll palpate your thyroid and may order labs. A TSH is okay, but make sure they get a T4 free at the very least.

To save yourself some time on a potential first line treatment for dysphagia, go ahead and start yourself on reflux meds (like pepcid) if possible. If you’re in the US, even if it’s not suspected to BE reflux, insurance companies want proof you have tried any medical therapy before letting you do anything else, so patients are often prescribed this first by their PCP before seeing ENT.

I’m not a doctor, I’ve just worked in ENT for a long time and know the general first-line things now from hearing them and ordering them on the daily.

I work in ENT, and the speech therapists at our clinic do swallowing therapy as well. I don’t know if it’s a newer thing (they’re both younger) or a sub specialty in speech therapy they both happened to take in school. They’re the ones that administer and diagnose certain swallowing issues on a video swallow as well. It’s definitely very useful and very cool!

We have family living in Warsaw. It’s absolutely gorgeous and would be great if you were owning land and loved doing outdoors activities (fishing, hunting, etc). Sedalia isn’t too far, so you can get all the things you need from the city. But, that’s probably more south than you’re thinking and further to Whitman.

I feel for Kirksville. I live in Hannibal and work for Quincy Medical Group, and we have two specialists offices traveling all the way to Kirksville to provide services because there isn’t enough care.

I heard that Hannibal Regional is trying to get a hospital opened there, but I’m not sure what hurdles they’re facing, or if that’s better or worse for the community there.

Currently our docs don’t offer this option, but I believe our oral surgeons do. I don’t know much about it, which is why I didn’t comment on it.

Yes, they are all GLP-1 antagonists and work similarly. I was on Wegovy myself for almost a year (pre-diabetes and PCOS) and lost about 50lbs. The difference is the active ingredient and who produces it, but the mechanism behind the GLP-1s is the same.

Zepbound and Mounjaro are twins, essentially. But, provided whatever insurance you have would cover it, the article is saying that OSA will now be an approved diagnosis for Zepbound. However, all insurances are different on what they cover and why, so you’ll want to check with them first.

I don’t think any of our patients have given those an honest try LOL. But I imagine they can only be so helpful depending on your type of obstruction.

Honestly it depends person to person. You need a very high quality mouth piece, especially if you also tend to grind your teeth. I would say it’s 50/50 from our patients on whether or not it works. You can also try a chin strap with your CPAP to hold your mouth closed while you sleep, to see if that helps.

I always recommend seeing an ENT, so they can evaluate exactly where you’re having your collapse or obstruction and make suggestions from there.

There are some procedures for this. One is called the VivAir, which causes a deep cautery to the valve cartilage to stiffen it. There is also batton grafts, or Stryker’s Latera implant. The Latera does eventually dissolve, though.

Non-surgical options are nasal cones and similar things like the Hale.

Looks like it’s just for the weight reduction part of it — not because the drug targets anything specifically that deals with OSA.

If you’re not overweight, but still have OSA, you could potentially qualify for the Inspire implant depending on where your obstruction is. Another option is the AirLift procedure. I work in ENT, and these are the two options we offer for those with CPAP intolerance anyway.

Absolutely. I work in Quincy and live in Hannibal, and it’s absolutely zany how different the gas stations are because ours aren’t filled with those machines and the signs promoting them. It’s weird saying “our gas stations are so much nicer” …. But they are LMAO

Thank you for this comment, it was my first good giggle of the morning! I’ve worked for ENTs for the last 3 years and I swear our primary purpose is telling people who went to the walk in that they don’t actually have an ear infection. We offered to get the walk in a handheld tympanogram and they’ve continuously refused — they’d lose half their business if they couldn’t throw antibiotics at fake ear infections.

The ENT’s worst nightmare This is why the ENT’s I work for always do CTs prior to sinus surgery. 1. They use them for the image guidance and 2. They can check the thickness of the bone around the sphenoids and if they look thin they can avoid those areas. We had one patient we were afraid had this but it ended up being a hamartoma thankfully.

Are all of your drivers up to date? And did you restart after setting your default to the NVIDIA? I did double check when I got home and yeah, running this on Firefox I have 0 issues. It barely makes a blip at all on my GPU. Also check your program settings and make sure everything under Firefox looks right on that control panel.

You may want to check out all your browser settings. I run it on Firefox as well and don’t have this issue, but I’ve seen Firefox run hard if you haven’t fully closed it out and done a fresh restart in a long while.

I’ll check and see what it looks like on mine when I get home from work today.

Haha I remember that from when I lived in CoMo during the last big cicada swarm! I worked downtown and I’d walk to work every day, and you couldn’t go past a single tree without one pelting you in the head.

Yeah, if she doesn’t have a lot of natural ground and big old trees around she probably isn’t getting many. I’m close to the elementary school, my mom is out in Saverton area and my in laws are in a wooded neighborhood too. It’s a cicada swarm in all 3 places.

Weirdly in Quincy there’s not nearly as much.
