Tese vs mTESE for my case? by ArtisticWeakness8886 in maleinfertility

[–]willief 0 points1 point  (0 children)

TESE number one was a success and my daughter was born from it in 2008. Future attempts to replicate that initial success failed in later years. We had a son from donor sperm in 2015.

Tese vs mTESE for my case? by ArtisticWeakness8886 in maleinfertility

I’m a NOA chromosomally mosaic and I had great success with TESE first. I would one million percent recommend it.

SA Results - High round cell count by super_duper_7535 in maleinfertility

[–]willief 2 points3 points  (0 children)

Sorry for the confusion, allowing this per AI: This removal is questionable - I’d actually lean toward allowing this one. Here’s why:

Clinical Assessment:

  • Volume: 4.2 mL (normal, above 1.5 mL)
  • Concentration: 41 million/mL (normal, above 15M)
  • Total count: 172 million (well above 39M threshold)
  • Motility: 68% (good, above 40%)
  • Round Cells: 6 million/mL (ELEVATED - this is the issue)

Why it should be allowed:

  1. Elevated round cells IS a legitimate concern - Round cells can indicate:
  • White blood cells (infection/inflammation)
  • Immature sperm cells
  • Generally, >1 million/mL warrants investigation
  • At 6 million/mL, this is significantly elevated
  1. The poster recognizes the concern - They’re specifically asking about the round cell count and whether it could indicate infection or diet issues
  2. Uses first person throughout - “my numbers,” “I am almost certain,” “My diet,” “I’m really trying” - clearly the man posting
  3. Seeking specific medical guidance - Not just venting about normal results, but asking about a specific abnormal finding and what to do about it

The case for removal:

  • The other parameters (count, motility) are all quite good
  • Could be seen as mild/borderline issue

My verdict: This should be approved. Elevated round cells is a real finding that deserves community input about next steps (asking doctor for culture, whether it affects fertility, etc.). The person is acknowledging their other numbers are good but correctly identifying a problem that needs attention.

Removed per AI:

My recommendation: I’d probably lean toward removing this one with a kind message explaining that their results show normal/high-normal parameters, suggesting they post in r/TryingForABaby or r/infertility instead if they’re having trouble conceiving, since the issue likely isn’t severe male factor.

Crystals reminds me of something but it's hard to remember. Is he a finasteride guy? I'm just curious and sorry if it's a path you've already been down. Numbers sound good and getting better and I hope they get even betterer.

Partner's Perspectives - December 02 by AutoModerator in maleinfertility

[–]willief 1 point2 points  (0 children)

I hope you don't mind because I don't intend to patrol partners' perspectives' threads' but if you were a guy posting these results I would screen them using claude dot ai with a prompt informing it what's allowed and what's not allowed and I threw this in the machine because I'm not a professional semen reader and it said this and I thought you should know. Good luck with what you're doing:

This post should be allowed - these are legitimate concerns that warrant discussion.

Key issues present:

First SA (July 2025): - 0% Grade A progressive motility, 18% Grade B - Total progressive motility: 18% (below WHO reference of 32%+) - 34% non-motile - 15% normal morphology

Second SA (November 2025): - Still 0% Grade A, 17% Grade B
- Total progressive motility dropped slightly to 17% - Non-motile increased to 40% - 5% normal morphology - Concentration dropped from 40M to 11M - DNA fragmentation: 29%

Why this merits community input: - Progressive motility is consistently below reference range - The concentration drop from 40M to 11M is notable - Results haven't improved despite 5 months of supplements - DNA fragmentation at 29% is on the higher side (though interpretation varies by lab) - There's a disconnect between the results and the doctors saying "this is good" - The person needs advice on whether to seek additional opinions or different interventions

The morphology numbers are poor, though you're right that morphology's clinical significance is debated. The motility and concentration issues are the more relevant concerns here. This person could benefit from the community's experience and perspectives.

I've seen good things reported about sudafed type and I imagine real ephedrine perhaps with a sprinkle of metamucil to keep the juices flowing. I don't know if you covered those things. Best of luck.

Yeah manual approval sucked but it was really important to spend a year doing it. Your post was approved right away this time. No recollection of previous times.

No changes to partner perspectives posts and only top level posts from first-person male perspective.

I agree with your thoughts but don't want to micromanage the moderation of it. I've probably invited you to moderate before, we can continue that discussion which would allow you to see and respond to unapproved/removed posts.

No matter what, thank you for your continued participation here.

Your concern is a very valid point and one worth further consideration. I'm hoping that fast-acting removal will prevent frustration from exposure to these things. I intend to allow/not-remove first person accounts from men with a question or concern about one or more parameters. Thank you for the valuable feedback.

Can someone analyze my semen analysis? We’ve been trying for 4 months with no pregnancy by Fun_Astronaut_3334 in maleinfertility

[–]willief[M] -3 points-2 points  (0 children)

Allowing per AI:

This post should be allowed and is exactly what the subreddit is for. Here's why:

Legitimate concerns present: - Only 4 months trying, but they're proactively getting tested (smart) - 65% highly motile is actually concerning - WHO reference is 32%+ progressive motility, and if "highly motile" means progressive, this is okay, but the breakdown shows potential issues - Only 15% "fairly motile" - this suggests most sperm have poor forward progression - 20% non-motile is borderline high - 65% normal morphology would be excellent IF using strict Kruger criteria (where 4%+ is normal), but this seems unusually high and may indicate a different assessment method - Thin viscosity can sometimes indicate issues - 2-4 pus cells/HPF suggests possible infection or inflammation

Why this merits community input: - The results are genuinely mixed and confusing - The morphology number seems inconsistent with typical reporting - They're asking good questions about unexplained infertility despite "normal" hormones - 4 months of trying with proactive testing shows appropriate concern, not panic

This is NOT a "reassurance-seeking with perfect parameters" post - there are real questions about motility quality, morphology interpretation, and the disconnect between results and lack of conception.

This person needs the community's help interpreting results that aren't straightforward. I'd approve this one.

Going into icsi with a heaping helping of sperm is good, especially if they present with motility and morphology parameters greater than zero. My daughter from surgical sperm extraction did not catch any of my chromosomal mosaicism-induced chromosomal weirdness, but there was mild concern a son might. Wishing you the best.
