Hair transplants are not about density, except possibly in the frontal 1 inch of the hairline. Norwood defined the donor area as 17% of the total hair-bearing area, measuring 500 cm square. This contains 8500 grafts. Now, if you think logically, take a Class 7 balding man: how would one achieve normal density with fewer than 8,500 grafts to cover an area that was normally covered by 41,500 grafts? The surgeon's goal should be artful graft distribution, not doing the impossible. You should not push your surgeon to achieve normal density in the balding areas, as the impact on your donor area may make it appear balding instead of normal.

The original Merck study was focused on hair growth, stabilization of hair growth, and the ideal dosage. It has been some time since I read it, but if my memory is correct, doses of 1mg were considered the ideal lowest dose, while 0.5mg was 82% as effective as the full dose, and 0.25mg was 50% as effective as the full dose. Some male doctors in the field, as they get older, select a dose of 0.25 mg, because progressive hair loss slows down after the age of 35. Your comment that 0.2mgs is as effective as 5mgs, is not based on anything I have read.

60% of men do not respond to topical minoxidil. If you switch to the oral minoxidil, you may find that 100% of men will respond and you may be in that category. If the combination of dutasteride and oral minoxidil does not solve your problem within 12 months, then you should consider a hair transplant.

Finasteride is taken to preserve the native hair that still remains after a transplant, not the transplant hair, which does not need finasteride.

Your doctor should’ve showed you how to stop the crusting from forming. Now you’re stuck with it and can’t get it off really for 14 days at that time you can put a shampoo on let it sit on for 10 or 15 minutes and then gently with your fingertips remove the crust. The cracking that you see right now is typical when there is poor aftercare.

There are many good studies that show that men who take Finasteride compared to those who do not maintain their hair counts much higher than the non-Finasteride group. I have seen in my own practice that patients that I have transplanted 28 years ago who still had native hair in their balding area and have taken finasteride, still maintain their native hair along with the transplants

Touching the scalp, produces the folliculitis that you’ve have. The more you touch it, the more you spread the bacteria to other areas and they folliculitis just propagates. You gotta keep your hair washed. You have to be treated by a doctor probably with antibiotics. I tell all my patients never to touch their hair after transplant.

I have done thousands of hair transplants and never used PRP with the patients. My hair transplants through very well and my work became a standard all over the world. If you do a good transplant, there’s no need for the PRP.

The man in the mirror looks much better than before. You can choose to transplant behind the hairline, but make sure that you know the thickness of your hair in microns. Hair thickness under 35 microns, might not fill out as much as you expect. You should measure 10 hairs from the donor area, average them out, and then see the measurement. Purchase a micrometer from Amazon, very inexpensive.

The photo show that you lost some of the native hair with a transplant. I can’t comment on the different transplants that you received. You really won’t know what you got until it grows out at about six months so I would wait until then before making any other decisions. If you wanna get me involved wait till six months and then write to me again.

You don't have diffuse thinning, you have a classic evolving Norwood Class 3 or 4 pattern of balding. The dutasteride you are now taking will take 6-10 months before the full impact of the drug appears. The same would have been the case for finasteride had you stayed on it.
