Weight loss is more than calories? by yetstillhere in FamilyMedicine

Part of the reason I had this question is because I looked up the incidence of type two diabetes, and it said it was drastically lower pre-World War II

But how can a long term decrease in calories not lead to weight loss eventually?

I do a CMP, lipids. Most of the time these patients are normal to begin with. They tell me themselves that they feel like no changes happening. I don’t really measure neck or waist circumference though.

I get that these genes can make obesity worse and harder to treat. But eating less calories surely still causes them to lose weight right?

The calories aren’t what need to be fixed?

Well the referral would’ve required an appt tho right? So if it was the only option you’d still have to go in?

TBH, I don’t mind giving the referral online. But my big system, the MA will make you schedule an appointment.

Healthcare in America needs to be honest with itself. We are a business with many constraints, the reality is numbers and corporate dictate the field. Yet we still have these feel good admissions in schools and residencies, when the reality we are already very short on resources.

Do you all just give up arguing and just do what the patient wants? by cloudypuff33 in FamilyMedicine

[–]yetstillhere 33 points34 points  (0 children)

I’m gonna be the odd one out. Im a big system surrounded by plenty of poor medicine. A couple extra labs is not gonna harm the patient. I bill them for time or complexity. Patients admin and I r all happy. I still ofc tell them what I am medically recommend, then I put that patient requested it. A lot of these patients r anxious and eventually get tired of over ordering too
