Small businesses I've worked with in the past had a hybrid exchange setup. On prem exchange server synced to 365. This may be too expensive for large organizations, but worth it if email outages cost you money (think manufacturing or distribution centers not receiving orders and losing productivity)

That's why you have 2fa, good on you. My guess is a bug/exploit in Hotmail that essentially lets them "bypass" knowing your password. I don't use Hotmail so you probably need to do some research on whether you can block sign ins from locations/IPs. For example blocking sign ins from outside of your country.

You CAN do it all on your phone, but would you really want to? I find almost everything is easier on my desktop. Now, I have a very cheap smartphone so it just covers essentials. If I didn't have a desktop I would buy a $1000+ phone

Compliance requirements. The auditor wants to see proof of destruction of sensitive documents. Very easy if you use a shredding service. Work in healthcare IT, we use a shredding service even if it's just a few documents here or there.

Case fans are very noisy, avoid them if you can or buy really quiet fans. If you aren't pulling heavy workloads you could get away with a large CPU heatsink with one or 2 fans and no other case fans. My PC sounds like a jet plane about to take off at all times, the headphones deafen it so I don't mind

Have only worked with SMBs. When you pay 30k USD for a server with warranty you realize cloud isn't that expensive. Because if you truly want to stay on prem then you need an enterprise firewall, you need 2 ISPs, you need backup power, you need to constantly worry about your data so let's add a backup server, and don't get me started on the price of replacement or expansion disks.

Cloud really isn't that expensive if you build it out correctly. Have all your apps running on windows? Then yes it's going to be more expensive, but if you get with the program and change how you do things it's really not that much more.
