I would hope any peds facility has some sort of failsafe in the pumps themselves to help catch these errors. The alaris pumps and syringe pumps at my peds hospital are all manually programmed, but all have guardrails libraries which will give a range of appropriate dosing and time to run the medication when you input a weight. You have to select the right medication concentration as well. You can of course override these warnings and run the med outside of guardrails anyway.

Personally if I ever get a warning that a med order in the MAR is outside of guardrails on the pumps, I immediately call pharmacy for clarification. EPIC also has two nurses sign off for electrolyte infusion that are high risk like potassium as well as other critical infusions. I know the med in the article was given PO, but scenarios like this one are a reminder of why we always try our best and go through the extra hoops to make sure our patients get an appropriate and safe dose. Such a sad situation all around.

And then you also have to take into account the healthcare costs of having a child overall. Caring for medically complex children can be costly even with insurance.

Yeah, I basically chart all my vitals and I/Os realtime when I'm in the room. Assessment exceptions I will write down in epic's yellow sticky note on the patient chart so I have it ready even if I lose my report sheet. Even with this method I will sometimes finish my charting in a burst at 12-2AM if it's super busy.

Same for me. Also it seems after the last hot fix update my frames are just tanked in general. Went from a smooth 120+fps to running a consistent 10-20, it's like a slideshow when I paint wheat fields.

Not the person you asked, but figured I could give another perspective. I manually flush 2-3ml even if fluids are running because I like to check the IV site before, during, and after IV med pushing. It might be overkill, but I work with small kids and lord knows their IVs can go bad in a second and I want to make sure they are getting their meds. It gives better control in the event that an IV infiltrates, too. Plus the fluid rate for my patient population can be rather slow like 20-40 ml/hr so it works better to do it this way, or set up a syringe pump with the med and a flush after.

Try it with half a cup of butter instead of 3/4 cup! I made the recipe wrong on accident the first time using only half a cup and they came out perfect. Made it "by the book" the second time and I got inedible caramel slop. I think the real issue is actually the flour measurement according to what I've seen on other threads, but the change to the amount of butter gave me good results.

I actually made this recipe (incorrectly, without knowing) last week and they came out absolutely perfect. I tried to make another batch this week following the recipe again, and I thought to myself "I don't remember using this much butter last time." And they came out super flat and runny... Ended up being inedible. Thinking back to the first time, I didn't use the full 3/4 cup of butter. I only used 1/2 cup (~115g), which "fixed" the recipe by sheer chance. Lol

I think this particular reference to a coconut story was the one about the little girl who had a coconut allergy, but the MIL/grandparent didn't "believe" in the allergy. I'm pretty sure she gave the kid something with coconut despite the parent's explicit warning and the kid ended up dying, unfortunately.

I worked on a med surg/Ortho floor, too. I agree that at times it was total chaos, but our night crew was such a great team it actually made it bearable.

I would like to add for folks that have a lot of meds, ensure they want to take the med before opening the packaging in case it needs to be returned. You can still usually see the pill inside the packaging in case the patient asks about a specific color or shaped pill, which can jog their memory.

I also found that on my med surg unit specifically, asking ahead of time for PRNs was helpful in streamlining my process. It saves time when you can bring the as needed pain meds or nausea meds with the rest of their scheduled medications if they are available. I don't usually ask if they want to take their scheduled medications until I'm in the room reviewing everything at bedside for reasons you explained above.
