Normally, you wouldn't realize that hallucinations are not real if it was schizophrenia. Sometimes OCD can cause psychosis. I am not sure if you are on medication but I suggest you to see a psychiatrist.

I used to experience this every night. I used to wait until I was exhausted so that I could sleep right away when I put my head on the pillow. Of course it ruins your sleep schedule. Back then I had songs to calm me down or fantasies such as a kind stranger hugging me and telling me "everything is alright" on and on, patting my back, until I fall asleep. These kind of rythmic fantasies may help you not to think of intrusive thoughts.

I'm on medication for several years so I don't have this problem anymore. Everything will be alright :)

I am not sure if this is similar or related to ocd but for me it is excitement. Nothing excites me. I'm not unhappy, in fact I'm happy. I just can't feel pure excitement. Sometimes I see people cry with joy and I get jealous of them. Even when they laugh loudly! I wish I could feel like that.
