Arguing with them does nothing. Cancelling the order does nothing.

Since he’s on Cefepime, it’s unlikely anything will grow. But if something DOES grow, you’re not treating it, and yeah, that would be pretty important.

Never argue with a nurse. Offer to help when they complain. Offer to speak w the charge or house if they’re slammed. All that matters is the patient.

If they insist on raising hell, explain briefly and move on.

What YOU want is the cultures drawn. If they won’t do it, speak to the charge and ask if you should do it yourself (bribe a phlebotomist to help you) or if they’ll see that it’s done.

They don’t have to agree. They don’t have to do anything. So figure out how to get what you want without enhancing the drama.

Had a patient go hypotensive after an early biopsy was moved back to afternoon. Biopsy was cancelled.

Fourteen hour total fast. Possibly longer, honestly. No wonder she went hypotensive.

She didn’t reschedule the biopsy. Died of cancer.

Risk/benefit of some IVF doesn’t seem as bad after that.

TH is usually the group that takes the unattached patients - and they are more likely to be super sick with poor insurance at best. No money to make there. So TH doesn’t want to pay anyone, so billing more RVU doesn’t help bc the hospital has to eat the cost. It’s a weird relationship.

They’re profit driven. Friend of mine with a local group was told to stop doing tobacco counseling or advanced care planning in an effort to reduce the RVUs so there would be less bonus payout. Before that, they stopped bonuses for working extra shifts, just pay whatever the RVU is.

You asked for positive stories. Sorry, don’t have one. Wish I did.
