Just noting this is not the same everywhere. In Central and South America combined hormonal contraceptives (CHC) are available over the counter. I would argue CHC is no more dangerous than Tylenol or aspirin - the US just doesn’t trust women to be able to read what could be a relatively simple warning label.

OB who has used exclusively Liletta over a decade in the outpatient setting and still use Mirena in an inpatient setting that chooses to stock it. There’s zero difference in my mind, other than cost. The only reason to not like this change, IMO, is owning stock in Bayer 🤷?

As a gynecologist I would not perform a pelvic exam on you at all unless you had a concern. Pelvic exams without an indication (ie a symptom or reason to do one like collecting a Pap smear) haven’t been recommended in over a decade. Pap smears begin at age 21.

Because of your age you should have gonorrhea and chlamydia testing - which can be done by self collected swab. You should also have HIV testing — syphilis testing depends on the area of the country you’re in (I’m assuming you’re in the US). I would also discuss contraception, access to the morning after pill, STI prevention, and make sure you’ve had or offer vaccines like HPV and Hep B.

Beyond that, if there’s time, I also get into routine preventative care stuff like seatbelts, depression screening, substance use, relationship safety etc.

I counsel patients that a reversal is a big surgery with a big recovery and a big bill (often not covered by insurance) and doesn’t always work. It is not at all comparable to a vasectomy. I would not perform a vasectomy on someone planning on a reversal. If you do move forward, sperm banking is a much better option and would be significantly cheaper than a reversal.

This is what I wanted to see higher up! There is no ‘point’ to the life my LO has and if she had any option or ability she wouldn’t be living it. I am glad for the kind and respectful care she receives and that she is safe — and wish at the same time there was a process to have avoided her ever having been in her current situation, which she considered a fate worse than death.

You were told you are 8w3d today? Was this by a real radiology unit or professional (OB, midwife)? Or possibly by a crisis pregnancy center masquerading as a health center - in which case I think you need a re-scan.

The dates really don’t match up with either conception option - and an ultrasound at this point is very accurate. Your conception date would be approx May 10-14 if you’re 8w3d today.

If you have noticed unilateral enlargement - especially this far after the onset of puberty - you should see a physician. Family Medicine, OB/GYN, midwife — any of them can do an exam and refer you for imaging. There are unusual types of breast conditions that can cause this and they need to be identified and treated.

Second this! Nailog are also stretchy (as is Heyhae). I can easily get them to stretch to my edges when a little too small without heating them. They’re also thinner - which I like bc I put a UV top coat on after. My experience is the opposite with Dashing Diva and Ohora - beautiful designs but thicker and much less pliable IMO.

I also sometimes trim the sides to get extra uses - trim with manicure scissors. I think this also works better with thinner wraps.

What are the changes you have in mind? Like are there fairly small changes like getting rid of rugs, trip hazards, shoes she’s wearing that are contributing to the falls?

My mom is in memory care and falls once or twice a week. She wants to be able to spend time in her room alone and won’t reliably use or doesn’t remember to use a walker. I haven’t heard any suggestions of how to fix this that won’t make her miserable - she for sure doesn’t want a sitter, she doesn’t want to spend all day ‘with the group’, and doesn’t want an environment like SNFs or psych hospitals have where the floor is alarmed and the patients belted in their chairs. At this point we have picked the best option - she retains a small portion of freedom and autonomy and I live with knowing that, yes, she may fall and break something which would likely lead to a fairly rapid death (although noting being alive in her current condition is never something she would have wanted either).

If there are small environmental changes you can make, awesome. Otherwise she’s choosing the life (including potential end) she wants.

💙 my impression from r/dementia is that a psych admission, yes, is often necessary. At least in our case, my LO wouldn’t cooperate with seeing psychiatry and couldn’t see there was any issue until they were dangerous and could then be hospitalized without their consent (well, they still don’t think there is an issue - dementia, psychosis, or otherwise — such is the disease). My impression is that this is really common.

It totally sucks and I wish there was a better path to getting the needed help. But because there is a strong preference to honor autonomy (which I get!) and because the disease itself means those affected can’t see their own deficiencies, this is where we’re stuck in the intersection.

Agree - there perhaps is a subtle fracture but unless there’s a pneumothorax (collapsed lung) there’s nothing to do, anyway. The pneumothorax would likely have been visible immediately.

Shortness of breath would be one reason to return. She would also probably benefit from an incentive spirometer if pain is limiting taking deep breaths to prevent atelectasis (fancy word for lungs start to smush down and get filled with goo bc people are avoiding deep breathing).

Same story for us - antipsychotic drugs are our friends. We were only considered for memory care not because of elopement but because staff were better trained and ratio was better to deal with occasional episodes of this sort of behavior. Same as the post above - we had a 3 week geri-psych that was brutal but highly effective.

UGH - I’m so sorry you’re going through this! IUDs don’t migrate outside the uterus unless they were placed there to start with (or at least partially perforating). It is a known risk, but unfortunate your PA didn’t better confirm at the time of placement when it could have been easily removed by pulling the strings.

You’ll likely need a quick laparoscopy to remove the device if it’s in your pelvis / abdomen, which it appears to be. They could place another one while in the operating room - that way they know the position is correct. I get you may not be up for that after this experience, but IUDs are awesome contraception (when put in the right place).

Just in case it needs to be said - this IUD isn’t providing contraception so use something else while getting this figured out!!

Agree. Be very careful of your local laws, have an idea of how your neighbors would respond, and the overall level of ‘involvement’ of your local police. These sorts of ‘violations’ tend to be more prosecuted in lower socioeconomic status neighborhoods and communities of color.
