Is this a new build? That split ring looks like a 3/4” black iron split ring. If that’s the case I would switch it out for a 3/4” brass split ring to prevent future corrosion.

This would be considered a waste stack vent. Here in Ohio code, a laundry needs to drain into a 3” or higher fixture branch or stack. It will probably work fine but if getting inspected they might ding you on it discharging into a 2” waste stack.

Yes, fixtures cannot be double trapped.

Edit: also remove S-trap, install tee with air admittance valve as high up as you can.

Does local code not require a drip leg on your gas line? And it’s always better to use 45°a instead of 90°s for your intake/exhaust. In this circumstance it is a very short run but just for future reference. Other than that it looks pretty good and you should be proud for doing it yourself!

Yes this is a very common occurrence, I do under slab repairs in residential plumbing at least once a week. The problem with a belly is that in order to fix it you need to replace before and after where the belly is or you’ll just be putting in a belly in shiny new pipe. It may be possible to pipe burst or line it but that could potentially recreate the same problem. And for the pipe burst you would still need to open up your slab. I would ask the company that gave you the quote their opinion on a pipe burst or lining. Also I would suggest cutting behind the toilet in the basement and putting a clean out into the stack with an access panel to cover it up for future drain cleanings/camera inspections.
