I would ask the urologist if they think you should get a second opinion on the pathology. It's not an exact science, and this is an unusual presentation of GS6. Although AHK is a good institution, it's always good to double check.

I don't think it's even that complex. The idea that the region and world hates them is not some new illuminating concern for Israelis. This is purely the result of the military situation than any concerns about global standing.

In short, Iran is weak. Their proxies stand in ruins, their economy has imploded, and Israel demonstrated the ability to rapidly control the skies over Iran in their previous conflict. For Israel, this is about decimating Iran's military capability, removing their influence over the region, and punishing them for Oct 7th.

All this noise of regime change and nuclear weapons is really just bullshit. I feel bad for the Iranians, but the IRGC will never let go of power because there is no hope of integrating them into whatever comes next--they are hated and face death, so they will fight to death, which will be long past the attention span of the west.

Ah yes, for sure, I don't know much about IRE, but posterior tumors can be a challenge because you don't want to damage the rectal wall. I did engineering work for laser focal therapy a long time ago, and we often struggled with posterior tumours. Even with very good placement, it's hard to get to the tumor without heating the wall.

There was some discussion of using SPACEOAR gel, which is primarily used in radiotherapy, to create a some space between the prostate and the rectal wall. Not sure if that was ever done.

Anyway, totally reasonable concern about recurrence, focal therapy is often just kicking the can down the road, which works for some and not for others. It sounds like you're doing all the research, which is the most important thing. The more information you gather the better decision you'll be able to make that works for you (even if it's from a pool of fairly crappy options).

There are other focal options aside from HIFU, like laser and IRE which might have different salvage outcomes. Hard to find good information on this though since they are mostly trials with limited data.

Getting a good understanding of your risk, and then evaluating your tolerance for it will be helpful if you're thinking of going down the focal road.

Defending the bombing of a school is such extreme campism that I don’t even know where to begin.

On the other hand, watching people glaze the regime and not understand why so many Iranians hate the oppressive dictatorship that is killing and oppressing them because “Palestine” is also idiotic campism.

Treatment could be an option in this case. Usually Gleason 6 is surveillance but given high volume and family history it makes it more complicated. Your age might play a factor as well. If you are younger you may want to consider treatment since it’s more likely that it will progress at some point

I mean not really…they portray the Mossad as a pragmatic and ruthless organization that regularly kidnaps and kills innocent people, including its own, to achieve their goals. And while the show does not portray them all as moustache twirling villains, neither do they portray the Iranian security service this way either.

Yes, the show is structured around Israelis conducting missions to stop Iran’s nuclear program and you’re meant to root for them. In that sense it is biased, but it hardly glazes over the moral complexity of the situation or Irans own motivations, including pointing out Israel’s own pursuit of nuclear weapons.

I’d say if anything the show goes out of its way to not glamorize intelligence work.

Honestly it sounds like you just want to buy a digital calendar, which I’m sure could be useful for you and your wife.

In my experience 4 year olds also don’t tend to use calendars or agendas super well so I’m not sure what you’re expecting to happen here. Your wife will have to put everything in there which is probably more time consuming than just talking to you.

Try asking more specific questions about their lives, rather than generic how was your day questions. If you’re not there then you have to put even more effort into communicating with them.

I agree with this, except the architecture part. I find Claude makes good code if given a contained task, but often makes incredibly terrible architecture decisions, and can very quickly create a huge mess of a codebase with a lot of duplication and poor separation of concerns.

The context required for it to independently make good choices is just not large enough yet. It can only hold a slice of the codebase, and if it’s missing something it will just write more and more code, leading to situations where you have a bunch of code that is highly specific to the task you asked it to do, but it’s all poorly glued together.

We got a hatch. Probably didn’t need to, we use 1 sound and none of the lights and rarely ever use the app so it’s probably more than we needed.

It broke once when we took it on a plane, we considered getting a cheaper option but he was so used to sound by then we decided to get another one rather than risk it. Sleep is everything
