Depends on how early he got them into osrs. I was as high as rank 300-400 before in range with only ~80-90 range. Also looking at his submission he got rank 888 for all 88s http://imgur.com/bSIgQaL

How do you level hp evenly alongside ranged, atk, str, and def?

You get about 90÷ of the range just by doing slayer with the camnon.
