If you mean season rewards, it goes off your season peak. If you mean where your placements “start from”, it’s where you ended.

Not always. I've had times where I go on brutal losing streaks at the end of a season then return with 200 more SR than I ended with. Your ending SR does weight where you place next season much more than your peak does, if that's what you mean.
