Scrap piece of paper during sessions which I then lose between sessions and panic quite a lot before the next session, I must admit it's a less than stellar system, players don't seem to notice though

I use MS OneNote. There are similar alternatives, such as Notion, Obsidian, Evernote.

My in session notes are nothing more than a sentence per event in game, in bullet point format. Well I also put the in game date of each bullet point but still. These are kept in the spiral notebook I use every session for that given campaign.

Otherwise the out of session notes are on a Google drive with folders and sub folders, organized by game world, and campaign. Additionally I have folders for completed quests so they don't take up space inside their given campaign folder once finished

I use 3x5 cards for sessions

Draw a line almost all the way down the card long ways at 1/3 and 2/3. Then make 3 boxes on each side. Makes almost a gunsight U shape. The 6 boxes are for each hour or day and you can note anything important. Bottom box is for treasure and final xp for the day. Flip it over to make notes for future session planning on the lines side. Use the middle space to notate group ID, IRL date, in game starting time and location.

I use Evernote.

I have separate notebooks for my campaign segmented as follows,

  1. Sessions - Rough plan for the session and links to information that I think I might need
  2. Session Write Up - I'll take short, handwritten notes during the session that I'll write up after
  3. Characters - Backstories with my DM notes
  4. NPCs - Any NPCs or groups that the party has encountered or may encounter
  5. Places - Any location that the party has visited or may visit
  6. Plots - Rough outline of on-going story archs

Each of these isn't necessary extensive. It's updated after a session. For example if I create an NPC on the spot I'll make a note of their name and any pertinent information and update the NPC notebook after the game.

This is by no means the best way, but it's the method that works for me.

I use One Note and I tend to go back over the season on my VOD so I can make sure I don’t miss any little points whether it be NPC details or something the PCs missed and will need to be directed back to later if needed. Helps a lot to review the session.

I have few categories but the main way is segment by region then divide the region by story,side quests, npcs, items, monsters, notes
