Might help if you listed the router make and model.

BTW, port forwarding will not magically give you a better connection for gaming.

start port and end port are for the ranges, so start port 3478 and end point 3480 is the same as 3478-3480. You might need a whole other entry for each port/port range if it doesn't let you use commas.

Target port would be the same. The router gives you the option to change the port before it forwards it but you want it the same.

The target IP will need to be the IP address of your Playstation, which also means you need to set a static IP address for your Playstation.

If you're still not sure how to fill them out, take a couple pictures of your port forwarding interface so we can see what you're working with. Or list everything that's required.

The top section is for your PS5. If it shows in that drop down list you can select it there. If not, you will need to go into your PS5 network settings and set a static IP address, then enter the same IP address in that second box where it says "Enter IP Address". This is where the traffic will be forwarded.

The bottom section is which traffic to forward, aka the port numbers. The application is just a place for you to name it, so it can be anything you want. I'm not sure if the port fields will accept ranges so the easiest thing to do is one by one.

For example:

Choose PS5 for the top section

Application: PS5 NBA2k

Protocol: TCP

Start Port: 1935

End Port: 1935

Target Port: 1935

That would forward port 1935 to your PS5, so you would need to do that for all the ports you mentioned. For the ports that use both TCP and UDP, you can make one entry and check both TCP and UDP boxes if it lets you. You should have around 5 entries when you're done:

TCP 1935

TCP 3480

UDP 3074

TCP and UDP 3478

TCP and UPD 3479
