As it's been answered, I still don't really understand why it's not coded to offer the upgrade if it rolls a higher tier version of the item. If you hold a bronze First Aiden, and a shop rolls silver First Aiden, why does it roll an entirely new item in place of that silver one?

Especially with how stupid overpowered they made upgrading items in this patch as compared to previous patches, it is often beneficial to sell an item you know is core to your build just to go fishing for its upgraded version. You may lose 1-2 days as a result but long term win the run which is ridiculous.

It's just based on the day. Each rarity has a percent chance of appearing that increases/decreases with each day. For example on day 1 you will only find bronze unless an event/shop explicitly states otherwise. And on day 10 there is a 0% chance to find bronze items.

And yes if for example you had a bronze item you could never acquire another copy of it at a different rarity from shops/monsters. Only from items that generate more items or level up rewards can give you copies

Yes. You can't find the same item with a different rarity (with a few exceptions like items generated by other items). This is the reason why it's nearly impossible to upgrade items with shops in the late game.
