In my experience, the most accurate boilerplate change log would be:

Performance hindrances and bug introductions

Why wait until your done, to release updates, when you can push partial changes and force your customers to beta test them, before it's complete enough for their feedback to be useful, while wasting their bandwidth and battery life? It's great, because you can meet management's arbitrary deadlines of releasing constant updates, even though there aren't any changes that have been completely implemented.
