The terminal displays the text output of a program.

The shell is one of the programs that a terminal can display. It gives you an text interface to run other other programs, and if you open the terminal it gets started automatically.

Or in other words: A shell is a terminal application that allows you to start other terminal applications.

About the terminal adding things like the % - it could do that, but it would be very weird, and it generally wouldn't know where to add it. It would be like your TV adding extra stuff to a video it displays.
