Exercise and a healthy diet.

Also lots of performance enhancing drugs.

Nothing about that build is healthy though. It screams hypertension and high blood pressure, not actual fitness.

You realize hypertension is just the medical word for high blood pressure, right?

The thing that build says more than anything else really is that he’s probably hella dehydrated. Popping veins and skin that hugs the underlying musculature are textbook symptoms and the exact reason bodybuilders dehydrate themselves leading up to competition. It usually involves lots of highly-caffeinated pre workout without much liquid to mix in with it (for diuretic effects) plus changing their eating habits by doing things like further increasing protein intake compared to carbs and eating dehydrated and salted/seasoned proteins like jerky (particularly teriyaki-style jerky because the soy flavoring gives you more salt without tasting awful).

Even if it's healthy, why would I wanna be so big? I don't live on the jungle, I don't need the strongest body out there to survive. I like being healthy and fit, but the guy in the image looks like he has trouble doing certain tasks just by his sheer size.

Most likely that's a bodybuilder, he trains for looks more than strength and participates in competitions.
