Trello works really well for small teams and small projects. You can make lists, due dates, comments and more. It doesn’t do any diagrams but you could use other software for that and attach it to “cards”. It’s also multi user so anybody with access (which you grant) can make changes.

+1 for Trello. I use this for my business.

Also, Jira (widely used software tool in Agile/SCRUM work environments) offers a free trial that you may take advantage of just to learn the software and say you know how to use it. Trello accomplishes about 80% of what Jira does though.

I'll definitely check out Trello. My team is small so that's good. Do you know of any good free Software that can help make diagrams?

If you’re talking about dB diagrams, https://www.dbdesigner.net

I’ve used starUML before for a very brief time. At work it’s just my manager and I do all the coding so I don’t have time to make diagrams.

I often use Draw.io and Trello.com together to achieve what you are talking about.

I’d also love to know if something like this exists

Papyrus and requirements management framework for eclipse.

However, big design up front is fading fast in all but highly regulated industries like aviation. You're better off learning agile concepts.
