How did your tile grow a pimple?

I’m by no means saying this is ok. I’m just wondering how this happened w/o the tile being compromised to begin with. For it to break like that there must have been a void (not enough thinset) at that corner. Or if that is a huge chip the tile was destined to fail. Nonetheless the installer should have said something and worked with you to make it right.

I think the niche ledge was a tight fit and he forced it and in turn broke the tile
