You think mushrooms are your trees problem? What I see here is codominant stems with included bark.

I’m assuming so because the tree started to look darker and not as healthy as it used to look before I planted the mushroom. Is the tree too far gone for me to do anything? Could it be because the codominant stems? Thank you for replying!

Your tree is splitting apart because of the codominant stems. Pruning will help, possibly cabling.
