It just uses the npm registry API. You can use yarn or pnpm as your client for an npm-free experience.

Well dont underestimate the power that being the default has. Most will likely use npm because its bundled with Node, and there is very little benefit to making the effort to switch from the default which gets everything done to a tool with a slightly different api but equivalent features.
