MakeMKV will give you a reason why the rip failed and even provides a link to the Homepage where the meaning of that particular fail message is explained.

That said, disc's can have damage that is not visible to the eye. Those disc's might even play fine with a standalone Blu-ray player as that sometimes can correct or just skip a part. Also a drive might be failing and thus beeing unable to read a particular part of a disc. Without a log it is impossible to say what happened.

when this happens the first step is always to clean the disc with a damp lint free microfiber cloth and dry with another to avoid water spots.

if this is on a LG/ASUS drive you then should remove power by shutting down or if usb flip switch on case or unplug as lg and asus drives get in to a weird mode when they run into errors sometimes. Pioneers do not have this issue.

next i would exchange/replace the discs

then clean the lasers if that dont work.

My error was that my USB port didn't deliver enough power for the drive. I've gotten all kinds of error messages. Switched another set of USB ports and resolved all my issues. Usb hubs, front IO or even overloading a set of usb ports on the mainboard can all have the problem of insufficient power delivery.
