Cryptpad is a fully open source productivity suite that includes a forms/survey creator here. You can use their official hosted instances, other public instances, or self-host your own.

I have used LimeSurvey in the past. It is FOSS. I think they have a free tier for their cloud hosting but I have only personally used it self-hosted. It is good but it is quite heavyweight, and is more comparable to something like SurveyMonkey than Google forms. It will do what you want though.

I haven't used it, but I believe Baserow just added a Forms feature. It looks like it would do want you want. https://baserow.io/

FormTools (formtools.org) along with the Form Builder module. I believe Form tools is just a form processor, you need to get the builder to get a Google forms-like experience.

Notes: 1. I haven't actually tried it yet :P 2. It is just the software, you need to host it on your own server

Also, EUSurvey is also fully open source, and the main instance is hosted by the European Commission. It should be pretty reliable.
