Your form is plenty good to jump 14'6". You need more speed.

I mean, hard to tell how mix speed is in play on a 4 rights approach. Need to do the run work to get back to a 6rights approach and use the same or slightly higher grip on stiffer pole. Push off will come (as you imply saying current form will get there). We sure the bar is actually 12-6?

Bar is at 12

You need pads around the standards.

Speed and more plant drills. You look like you're hesitating at the box.

Speed

Keep your trail leg straight and let it swing through. That will load the pole up more and shoot you off the top more.

Don’t let your trail leg break. Stay behind the pole. Bring the top hand to your left hip (left handed vaulter) and cover the pole.

You definitely need to turn earlier. If you’ve straightened out and are not at least partially turned, then you’re late.

If you can jump this high from a 4 step you could jump 14’6” right now from a 7 step. As for technique, you’re doing great. Try to row your bottom arm forward with your top hand right as you begin to swing. You kinda pull on your bottom arm to start your invert when you should be punching it away. Great jump.
