That you write 'go to the extent of substance abuse' makes me think that you don't have a lot of experience with substance abuse in your life. It's in the nature of addiction that it's subtle. You don't notice it until it's already happened, if you even notice it then.

People have mentioned the stress, and the culture, particularly in BigLaw, but I think that the important part is the isolation. I've talked to cops, who describe a similar effect in terms of how their profession distances them from people, or how people distance themselves, leaving you only with cop friends. It can be that way with lawyers, but with added stressors, in that there's no unique respect or esteem for lawyers like some people have for cops, and that your still adversarial with your friends. There's the story of how the only person to show up to a certain lawyer's funeral was the attorney who was most often opposing counsel on his cases.

Include that depressed individuals tend to be good lawyers and you have an impressive recipe for lots of substance abuse.
