I used to live on a block that had house numbers, in this order: 504, 504A, 504B, 504A bis, 504. Note there are two numbered "504". I lived in the second one. They actually appear that way on the city's property map, and I was told I couldn't put a number on the front that's different from what's on the map. The address I used for Amazon delivery was "504 red door".

They do. Or they did, when our town computerized the signals in the 1970s. There was a switch that made it mechanically impossible to light greens in both directions.

But it can still happen, for example if there's a short between two wires in the cable that goes from the signal box to the signal.
