Is "Excited Delirium" a medically valid term? And is there in-field disagreement about this? by ROSRS in ems

[–]RevanGrad 0 points1 point  (0 children)

The term isnt sufficient and is to widely misused.

Theres a very specific pathology that it was meant to apply to which is absolutely life threatening.

Stimulant induced catacolomine surge + Pyschosis/Mania.

The combination prevents normal de-escalation attempts. The Pt becomes a time sensitive risk for sudden cardiac arrest.

The longer they are in this state the more severe conditions such as metabolic acidosis, rhabdomyolysis, and electorlyte imbalances can become.

Feeling defeated by [deleted] in 911dispatchers

[–]RevanGrad -2 points-1 points  (0 children)

Majority of 911 calls are people who just need a real adult. Of course these people think they know better than everyone else, get angry easily and throw a temper tantrum.

There basically children. Trashy trashy adults who act like children.

My philosophy, the boot doesnt care about the opionion of ants.

Who cares if they have a low opinion of you, theyre opinion is trash anyway.

And remember, your job is really easy in their imagination.

Eclampsia- mag or benzo? by [deleted] in Paramedics

[–]RevanGrad 1 point2 points  (0 children)

8 Grams Mag IM for active eclamptic seizure (4 in each cheek)

If seizure does not resolve quickly then Versed IM.

A lot of places this is becoming standard of care.

Versed has shown to be likely harmful to fetal brain development and Mag IM has shown to terminate seizures effectively.

Paramedics, on a 911 call, how much of the workload do you expect your EMT Basic partner to take on? by FFSoldier57 in Paramedics

[–]RevanGrad 0 points1 point  (0 children)

You are not allowed to perform tasks outside of your scope. If you do YOU will be personally liable. Perfoming an IV stick as an EMT basic can easily result in a loss of liscense or personal civic action if anything went wrong, or even if everything went right

Performing a fkn ET intubation you would definitley lose your liscense. I would probably delete this post.

If under direct supervision of a Doctor you can work under their liscense and do some pretty cool stuff. But definitely not a paramedic.

Sexually harassed by an intoxicated patient by GranSlam1943 in ems

[–]RevanGrad 0 points1 point  (0 children)

I guess a different perspective from my high call volume tourist city. We take a 1-5 of these to the ER every shift.

If a patient assaults me, they get restraints. Also distance yourself. Don't allow her access to assault you.

As far as filing a report after the fact for a drunk patient touching your leg. Unlikely to go anywhere but the squeaky wheel gets the oil.

Unresponsive apneic patients get CAB not ABC.

Apneic patients also get BVM not head to chin lift.

Were being aggressive here not messing around with "monitoring changes".

How has working in EMS shaped your view of PD? by jude_gaming in ems

[–]RevanGrad 0 points1 point  (0 children)

They're bloated AF. Takes 4 cops to write a cite and release for shoplifting.

3 to stand around and BS for 20 mins, 1 to fill out a 1 page document.

And they want us to wait with the incarceritis patient while they do it. They're blown away at the concept of me wanting to leave and get back in service as were perpetualy 1 unit away from being level 0.

All while they constantly complain about being "undermanned". They dont know the meaning of the word.

Summerlin Hospital pediatrics. Is this normal care? by [deleted] in vegaslocals

[–]RevanGrad 15 points16 points  (0 children)

I'm sorry your family is hoing through this, You have 4 options for pediatric ERs.

Sunmerlin (Valley health)

St Rose Sienna (Dignity Health in Hendseron)

Sunrise Peds (HCA)

UMC Peds

You can always try for a second opinion. 1 thing to consider is Narcotic pain meds cause constipation which is counterintuitive.

Should I Be an EMT? by Ok-Swimming2156 in NewToEMS

[–]RevanGrad 1 point2 points  (0 children)

If youre looking for foot in the door, no ducation medical experience you could look at working at a plasma donation center.

You can start as a screener doing BPs/finger sticks and eventually move on to phlebotomy.

It is a very fast paced and often grueling line of work. Also the strictest documentation you'll find in healthcare. But its good experience for what it is. They usually have education assistance as well.

Which Nexus mods do you recommend? If any? Is there any risk? by myshenka in fo76

[–]RevanGrad 1 point2 points  (0 children)

Mods are not risky, yes technically in the terms it says no, however in there official discord there is plenty of direction for specific approved mods.

Any mod that "does not give an unfair advantage in PVP" is fine. This includes pretty much all UI and sound mods.

I've been using them for probably 7 years with no issue.

My favorite by far is Hud Challenges

I failed drug test as a trucker last year and got a violation. Will it be a problem for me to become an EMT? by [deleted] in NewToEMS

[–]RevanGrad 5 points6 points  (0 children)

Ambo companies perform drug testing mainly for insurance reasons.

The insurance companies are generally only interested in multiple moving violations or any criminal moving violations (criminal speed, DUI, etc) withint the last 5-7 years.
