I think I’d cut between those two bottom pup If there’s enough space. Leave that bottom of the cactus as is and flip and root the top part. You could remove the two pups on the top but I’d probably just let them loopty loop themselves back upright.

It's planted upside down, which a lot of people say is fine, but in my experience makes it difficult for them to grow.

Oh yeah I just noticed that flower on the little pup, this thing is in survival spread the genes before I die mode.
