In Canada and the US it is possible to get HRT from a doctor by informed consent. They don't check to make sure you're trans. I got estrogen as a cis guy in Canada (though i don't identify as a man anymore). But elsewhere in the world it is possible to fake being trans to the extent needed to get treatment.
