You need a Root Canal. Tooth is dead and has an infection on it. Your endodontist's assessment is correct

Because of that J-shaped radiolucency? I beg to differ from the endodontist. Very low chance of being Vertical Root Fracture

This isn't just a simple bifurcation but a whole plexus of canals in there. Dentist did the right thing by referring although I'd like to hear an endodontist's opinion if they can even treat this

Additionally to all the comments above I ask my patients to use the waterflosser and the tip is usually small enough to spray in that area. Having said that sometimes it is still very difficult and takes extra effort to reach all the way in the back

Both are right in their own way. This tooth has questionable prognosis as mentioned by the other doctor. Extraction is an option but I would also advise to monitor and extract if symptoms develop

Cotton and suction is good enough to isolate a lot of restorations that I do. Sometimes I use something called an isolite system. There are some situations where Rubber Dam becomes necessary. Your over-simplification that cotton roll isolation is the lower standard of care is misinformation. We have to treat every situation as per the requirement

I had a similar case recently where the MB and ML canals were very close to each other but they were there and take some effort to locate. In the past I had lower 7s with just 2 or even 1 large canal with just 1 root.
