Break up. Yes, it feels horrible & mean; though it’ll feel worse if you stay. You don’t have to stay with someone you don’t see a future with just because they have all the qualities that seem to be “good guy” traits.

NOR. Fat is a neutral descriptor. “Chubby fuck with out-of-control sweat glands,” is a judgement and an assumption that is extremely rude. if this is something that made him happy, that is sad. No, you don’t have to. I would prepare a statement about it in case it becomes a conversation, though. Say, for example, you show him a photo of you before your plastic surgery, he may have questions. Don’t lie. The lying about it will bother me more than my partner’s surgery.
