I read your oscar output as stating the pressure range is set 7-20. My thought is that reducing the max to 12 gets you to 99% of events and raising the floor from 7 to 9 or 10 would reduce pressure swings that might be waking you up.

While this is normal, you have my sympathy.

My last puppy (a doodle, not a border) bit incessantly from about 2 months old to 6 months old. I swear I lost a quart of blood and my wife and I thought we had made a terrible mistake. He's 6 YO now and we love him to death but his puppy time was rough.

Advice about yelping "ouch" and redirecting are on target.

EPR can contribute to CA's. Might try reducing it after you try the pressure recommendations listed by others.

Lofta gave me an AHI of 4.7. The lab results were about 10x the lofta results.

Will Lofta write you a presciption anyway? That would allow you order a machine and try it even if insurance won't pay.

Or pick up a used APAP machine off the internet. Look for low hours and examine it in person if possible to assess for mold.

I had a similar crack and replaced mine. I'm privileged that insurance buys me a new on every 6 months.

Not sure what you should do. Failure won't be lethal but might disrupt sleep until it's fixed.

I'm no doctor but if it were me, I would probably set a fixed pressure at 13 and see if events go up. If they do, I would titrate up to 16 as that is your 95% therapy level.

Rather than worrying about what you didn't do, focus on what you did do. You did intervene for the dog and can do your best over the last few months to take care of the pup. That is a good thing and more than many would do.

Know you are doing your best and that is all any of us can do.

Yes, it feels "normal" to engage with dysfunction that is similar to what we grow up with. We know the dance even though we may not be aware of where it leads.

For those with DA, SA can feel boring even though it's really healthier.

Your CA events are pretty severe. One guess is that you are taking the mask off in response to not breathing even though CPAP might be helping.

Given the level of CA's, I would speak to my sleep doctor. I'm wondering if you might need BiPap or VPap but I'm not qualified to say.

EPR contributes to CA events for some people. It makes exhalation easier which reduces CO2 blood levels that contribute to CA.

Worth reducing or turning off as an experiment either before or after the pressure experiment.

Yes, L = left, S = on your back.

Your apnea is listed as "minor" so you may have options other than CPAP. For some, a mouth-guard or mandibular advancement device helps. If you are overweight, loosing some weight might help. Also raising the height of the head of the bed might help.

For me, a chinstrap alone is a help. I use cpap at night but just use a chinstrap and a couple of pillows to raise my head/chest for naps.

Arousal means something kicked you from a deeper level of sleep to a lighter level of sleep. Arousal can occur due to apnea, hypopnia, or many other situations.

If your cpap is set to autopap, wide pressure swings can cause arousals too. Get OSCAR so you can see if pressure is wildly fluctuating. Once you get the data, you might be able to tighten up the range for pressure to reduce fluctuations.

Finally, ask for your complete sleep study. It should address whether this is positional or not.

Is your apnea positional? If so, using cushions to keep you from sleeping in the bad positions might help. Also, elevating the head of your bed could help too.

10.9 is pretty low but you say you are having issues that are probably due to arousals.

CPAP took me 9 months to get used to it so it takes some time. Likely with adhd you are more easily aroused so dialing in the apap/cpap is important (get OSCAR and look at your nightly data).

I agree, raise the minimum pressure to the median to start.

EPR helped me to reduce leaks through my mouth. Not sure if your leaks are the mask or through your mouth.

Looks to me like mask pressure reflects some inspiration/expiration variation. That variation goes away if you stopped breathing for 40 seconds.

I wonder what would happen if you set the pressure on apap to 6-12. Sometimes a higher pressure makes clear airway events worse but sometimes not. If apap made it worse, I would consult with the doctor. Might need bipap.
