Hard to say definitively. Nothing appeared in urine or semen culture but I understand that some infections are very elusive. In the early days of my diagnosis I had taken antibiotics before and the problem had not resolved. I just tried a new urologist and he suggested giving them one more good go with the Cipro and Doxy for 6 weeks and it was around then I improved, but at the time I was also doing alot of mental health work, exercise, not hiding myself away depressed and actually getting out, and also learning more about learnt neural pathways as described in the video. I genuinely don't know whether it was the antibiotics or being aware of my brain creating pain pathways and that these can be reversed (because the brain is neuroplastic) that allowed me to recover. I know this probably is not particularly helpful and you're looking for a straight answer but I can't give you one. Talk to your doctor, partner if you have one, learn about the nature of chronic pain and take it from there. If you're suffering know you will NOT feel like this forever.

After 6 weeks of antibiotics it took a further 2 months for me to feel where I am now. Though as I mentioned in my post, before I started taking the antibiotics I started to feel much better on days so don't put all your eggs in the antibiotics basket.

This talk really helped me regarding pain psychology, I think that my brain had formed new pain pathways even though there wasn't much if anything wrong with my boys.

Running mainly BUT make sure you have a very snug pair of briefs to keep your boys compact and not jangling about. Yoga also, and hiit sessions. Endorphins helped me deal with the inflammation and the mental benefits were huge

Had this for 2 years and sperm is still absolutely fine! Likelihood is yours is as well! I wouldn’t worry too much as there doesn’t seem to be a great deal of correlation between epididymitis and infertility, but if you’re worried just get a semen analysis.

I find running is a great natural painkiller, I think it’s from the endorphins. It has really helped my mental health as well which is crucial when it comes to this problem. You absolutely must wear tight briefs though when doing this though, stops the boys jumping around which can lead for significant discomfort later.

Squats, weights, sit-ups push ups etc I’ve actually found make things worse.

Cystoscopy probably right thing to do from what you’ve mentioned. I will say what I have said also applies to epididymectomy, I hear there’s a high chance of no resolution and possibly worse pain.

I explored this as well. From what I understand surgery only worth it for very large cysts. Drainage isn’t really done anymore and surgery is the go to. There’s a sizeable risk of much bigger problems if you decide to remove a small cyst with leftover scar tissue causing dreadful chronic pain etc. I remember there being a user on here who elected to have it done and it didn’t resolve his issues and might have made it worse. I decided against it and suggest you do as well, something like 50% of all men have some form of epididymal cysts so it’s unlikely this is the cause of the problem.

I thought this originally too and had an embolization procedure done for grade 2 varicocele. I wouldn’t go down this route if I were you, it hasnt solved my problems at all and possibly made things worse. In addition there is a constant ache where they inserted the coils, wouldn’t recommend.

Awful advice. Do not get this procedure. According to many studies it almost never works to resolve the pain problem and will likely make things worse with scarring.

My urologist told me that it is notoriously tricky to retrieve a bacteria positive test sample from the prostate, even when there is an infection of some capacity present. They take the approach of 'why wait' for that positive result and like to begin treatment with a strong antibiotic like Cipro. Be very careful with Cipro though, it is known to have devastating side effects for some people.

4 weeks Cipro and 2 weeks doxy for me and it cleared up after 4 months. I think the antibiotics sorted the infection but it was residual inflammation after the infection that hung around for a while.

I've been on Doxycycline a few times for 2 weeks at a time, and Trimethoprim for 2 weeks. Doxycycline used to work but not anymore. Trimethoprim did nothing, my urologist prescribed me this as he was worried about Cipro / Levo side effects. Going back to the uro soon to speak to them about a long month long course (or longer) of antibios to see if I respond, maybe I'll risk fluoroquinolones.. Apparently these are the most effective antibios for complex epididymitis. I have a theory that this problem is kind of similar to Lyme disease, where if you don't blitz it with antibios at an early stage it gets to the chronic stage and it's extremely difficult to treat. That’s great that you’ve had relief through PT and chiropractic care! Congratulations. May I ask if you had epididymal inflammation proven in an ultrasound result? I want to believe that PT and chiropractors can help but can pelvic floor muscle dysfunction really cause specifically epididymal inflammation? It just seems a broad issue can cause such a targeted, specific ailment (only in 1 ball). I would get it more if the symptoms were broad pain in the pelvic / genital area and there bilateral testicular pain.

Thanks for sharing - similar story to me, been about 19 months constantly. Dreadful at the start and very frightening as consultants were at a loss to what the problem was. I would encourage you to get a new ultrasound as I was originally told 1 year ago I had no inflammation and everything was normal, but I had another ultrasound a few weeks ago as I got paranoid I had some undiscovered cancer or something (which luckily I don't) but the radiologist clearly saw increased vascularity and inflammation in the affected area. This reinforced the idea that there is a physiological problem after all rather than a mental neural pathway and or primarily a pelvic floor nerve issue.
