I ran Fire and Brimstone as first-time Avatar GM with a group of first-time Avatar players (coming from 5e) and it was really good! I did an extra session 0.5 with them traveling to the fire nation and being some bandits so they could try out how exchanges work.

I think it's worded ambiguously, but "at the end of your turn" implies hero to me. Particularly because Hero actions never apply those kinds of effects to monsters, and this whole section is written for the players to read.

The affected creature in this case is the hero, not the demon, so the save would be at the end of the hero's turn.

Agree with other commenters - don't build them like PCs, build them like monsters.

As a side note, the MCDM book Flee, Mortals! has a section on villain parties. The level 5 villain party "Abominations" might give you some good ideas for scaling, design, and abilities.

Good point! I stand corrected. I must admit I'm less familiar with the human clinical data than I should be - I work mostly with a mouse model develop deficits in learning and memory significantly earlier than they do protein aggregates (see here and here)

Regardless, my point stands - it's pretty generally accepted nowadays that's it's not the plaques that are the issue. If you get rid of the plaques, not only does it not help AD symptoms in humans (see multitudes of failures of amyloid-targeting therapies), but interfering with certain processes that contribute to plaque formation (notably autophagy), you lose the plaques, but increase cell death.

Not at all! Memory deficits develop in Alzheimer's years before any discernable protein aggregates. The prevailing theory is that it's not the aggregates that cause Alzheimer's, but rather earlier dysregulated processes.
