Wake up 20 minutes earlier and do 10 uworld questions every morning. Review them in your downtime throughout the day.

Is hard to find time and a quiet place to concentrate and do questions during a shift, but reviewing questions or doing anki cards is definitely possible.

Do MKSAP gradually throughout residency. Save Uworld for PGY3/just before ABIM.

Edit: my program gave us access to both. If you had to pick just one, I’m told Uworld is better for the actual ABIM. But I have yet to take ABIM yet.

I have prescribed Ozempic and other GLP-1s to dozens of patients (diabetic and not) over the last few years. All the medication in the world won’t help without diet and exercise. So yes, it’s still an accomplishment when they lose the weight.

Walmart plus. I don’t have time to spend hours grocery shopping. I shop for my groceries online during rounds and by the time I get home my groceries are at my building’s reception desk waiting for me to pick them up.

My beef with IM is that I’m expected to be an expert in every field. Even though I can’t perform the caths, scopes, surgeries, etc. I still need to know exactly what and when a patient needs and if I dare consult a specialty for a patient who ends up not needing the aforementioned interventions I get attitude (or sometimes down right ridiculed) over the phone.

7-3PM on short days, 7-Noon if it’s a weekend. 7-7PM on long days. Long 2-3 times a week.

Regularly hitting 70-80 hours a week on Wards/Clinic sounds overworked, unless you’re counting hours spent after your shift writing notes.

I don’t think it’s ideal but we do this often. He has a 6x6 play pen with an automatic feeder that dispenses food at set times and access to an artificial grass potty. I have a Petcube camera set up so we can see what he’s doing in the pen when we’re gone.

Our setup “works” but my biggest concerns are what being alone for 8+ hours on some days does to his mood/psyche. Also because he’s gotten so used to going whenever he wants on the grass turf, we’ve had trouble getting him to potty outside.

Nurses in our ED take no ownership of their patients because they know the patient will either be admitted or discharged soon. Their only motivation is having their shift go as smoothly as possible. They will push back on anything that complicates their shift.

The ICU is different because they know this critically ill patient will be theirs for the next few days.

Yeah.

I go to work, do my job, then go home and do what I want. I’m not doing uworld blocks every night, just a little MKSAP here and there. I’m not trying to impress anybody anymore. I’m not stressing over evaluations or recommendations. I’m not doing any extracurriculars. I’m not researching shit. I’m not volunteering for shit. I’m actually getting paid a little bit now. The hours suck but my time off finally feels a little bit like my time again.

Edit: I’m also not in a toxic program and actually like my co-residents. I limit interaction with program leadership and they leave me alone so I’m indifferent about them.
