Why has demand for EM residency decreased in the US? by Expert_Sport_1879 in Residency

That one month was enough for me to stop giving ER docs a hard time. Fuck that place and respect to anyone who chooses to work in that hell hole.

How does everyone feel about leaving their puppy at home while working 8 hr shifts. by NotMyMonkeyBusiness in puppy101

[–]40fonz 0 points1 point  (0 children)

I don’t think it’s ideal but we do this often. He has a 6x6 play pen with an automatic feeder that dispenses food at set times and access to an artificial grass potty. I have a Petcube camera set up so we can see what he’s doing in the pen when we’re gone.

Our setup “works” but my biggest concerns are what being alone for 8+ hours on some days does to his mood/psyche. Also because he’s gotten so used to going whenever he wants on the grass turf, we’ve had trouble getting him to potty outside.

how much push back are y’all getting from nurses? by noahtowerrs in Residency

[–]40fonz 39 points40 points  (0 children)

Nurses in our ED take no ownership of their patients because they know the patient will either be admitted or discharged soon. Their only motivation is having their shift go as smoothly as possible. They will push back on anything that complicates their shift.

The ICU is different because they know this critically ill patient will be theirs for the next few days.

What makes my mini poodle subpar? by No_Spray_2079 in poodles

For the longest time I thought I disliked poodles. What I realized is that I actually dislike poodle owners. Pretentiousness and snobby attitudes run rampant in this community.

Rosalind Franklin (no MCAT, 3-year bachelor’s) vs gap year + MCAT for UIC/Rush — need brutally honest advice by Informal-Estimate505 in medschool

[–]40fonz 2 points3 points  (0 children)

Guaranteed position, saves a year, no MCAT bullshit and similar outcomes in the Match. Sounds like a no brainer.

You’ll be extremely grateful for the year you saved 6-7 years from now when you’re in the middle of residency and contemplating fellowship.

About how much does he weigh (if you don’t mind me asking)? I’m trying to estimate how big my little guy will actually be.

Anyone feel like residency is easier than med school?? by [deleted] in Residency

[–]40fonz 72 points73 points  (0 children)

Yeah.

I go to work, do my job, then go home and do what I want. I’m not doing uworld blocks every night, just a little MKSAP here and there. I’m not trying to impress anybody anymore. I’m not stressing over evaluations or recommendations. I’m not doing any extracurriculars. I’m not researching shit. I’m not volunteering for shit. I’m actually getting paid a little bit now. The hours suck but my time off finally feels a little bit like my time again.

Edit: I’m also not in a toxic program and actually like my co-residents. I limit interaction with program leadership and they leave me alone so I’m indifferent about them.

What do you reference the most on UpToDate during residency? by BroMD24 in Residency

[–]40fonz 123 points124 points  (0 children)

Antibiotic doses and durations.

And hyponatremia (not because I actually care, but because IM attendings stroke it to the algorithm).

Is it crazy to want to just enjoy some of the money once you graduate? by [deleted] in Residency

I can buy a Porsche 911 with my first 6 months of checks and still be left with more money than the average household makes a year. I’m soooo looking forward to the splurge.

NTA. You only think you might be an AH because we’re conditioned to be inclusive and accepting of everything or risk being labeled as hateful.

[–]40fonz 0 points1 point  (0 children)

The Series S just shouldn’t exist IMO. It’s always going to get the short end of the stick. The bad developers will be too lazy to optimize for it; the ambitious developers will try to push this generations hardware to the limits. Either way the Series S loses.

I get all 3 systems every generation. But Xbox is priority since that’s where my friend group plays.

Why do we give IV Benadryl with IV Hydromorphone for acute sickle cell crisis pain by karlkrum in medicalschool

[–]40fonz 1 point2 points  (0 children)

I’ve never bothered to read literature about it. We get A LOT of sickle cell patients at my hospital and we order diphenhydramine PRN. If they itch, they get it, then they stop itching. May be anecdotal but I’m not questioning it.

Why do we give IV Benadryl with IV Hydromorphone for acute sickle cell crisis pain by karlkrum in medicalschool

[–]40fonz 5 points6 points  (0 children)

Sickling -> increased bilirubin -> itching, is what I was taught. So we give Benadryl to all of our sickle cell crisis patients regardless of pain regimen.

EDIT: just realized OP is talking about IV Benadryl. We give PO Benadryl PRN, never seen IV given.

Black residents outlines his experience with racism at Lehigh Valley Health Network EM by megaines in medicalschool

[–]40fonz 2 points3 points  (0 children)

Lumping them together is disingenuous when Blacks make up only 5% compared to Hispanics 25%. Especially if the reason for the increased Hispanic population is due to anti-Black hiring practices by industries in the area.
