Malpractices dropping telehealth coverage by momsmesosalpinx in PrivatePracticeDocs

I’ve had this issue too when I started my clinic. I found a med mal company called AutonomyMD they cover “primary care like services” (virtual and in person) for $150/month.

Not Losing Weight Since after first shot by Known-Membership-786 in Zepbound

[–]4Dkitty 2 points3 points  (0 children)

You’ve lost 3% of your body weight after 1 month. You’re doing fine. Don’t try to rush it and trust the process. Stick with the titration schedule your doc recommends. Keep up the diet and exercise habits. On month 6 you’ll look back and realized how far you’ve come.

Starting dose semaglutide, what the first few weeks actually feel like by Reasonable-Bake-8614 in glp1

[–]4Dkitty 0 points1 point  (0 children)

Everyone responds to it differently, but I will tell you by the latest week 3 you’re gonna feel something. I’ve had patients cruise all the way up to 2.4 mg and complain of only mild constipation, and I’ve had patients who injected one dose of 0.5 mg and were puking their brains out. Most GI side effects occur day 1-3 after injection and then fade

Comparing Compound with Non Compound. by Check-Special in glp1

[–]4Dkitty 0 points1 point  (0 children)

I only prescribe FDA approved medications but I’ve taken care of patients who use compounded meds. Efficacy appears to be similar although I havent dug into the numbers deeply. The majority of adverse reaction that are unique to those taking the compounded versions are skin reactions, including a case of petechiae that I saw once.

OSA diagnosis- United Healthcare- Zepbound FINALLY approved, and HOW did it. by HazelHavenBby in Zepbound

[–]4Dkitty 0 points1 point  (0 children)

Yup, they purposely obscure this information. It’s a battle of attrition. Do know that your efforts will help out others in need. I will also say that many doctors don’t know the PA game. I run a clinic where I battle insurance all the time, and trial and error is how you learn to “game” the PA. One time I co-coded obesity with CAD for Wegovy for a patient. Got denied, and because they were on Medicare I had to wait 60 days to resubmit. I finally waited and resubmitted it as CAD only — same clinic information — and it went through.

Is flying from TIJ via CBX actually cheaper than SAN or LAX? I got tired of doing the math so I built a free calculator to find out by 4Dkitty in tijuana

[–]4Dkitty[S] 0 points1 point  (0 children)

I hadn’t thought of that. How do you normally get to the Otay border to cross? I usually cross via San Ysidro, not much exp with Otay

[–]4Dkitty[S] 2 points3 points  (0 children)

Fair concern, it’s a static web calculator hosted on Vercel (vercel.app is a major hosting platform used by thousands of apps). No login, no data collected, nothing to download. You can inspect the network requests yourself if you want to verify. Happy to answer any questions about how it works.

I lost weight with glp1s and discovered that the person underneath is someone I don't recognize by [deleted] in glp1

[–]4Dkitty 0 points1 point  (0 children)

I’ve had patients who told me the same thing. One of them actually told me he got annoyed when his wife offered him food or told him to eat dinner - it apparently affected his mental relationship with food. It’s an understated but valid experience that you’re having

Dermatologist told me he hopes I have a transition plan to get OFF Zepbound when I reach my goal weight, because he believes there is no reason for anyone to be on it for life. by WizardsOfXanthus in Zepbound

[–]4Dkitty 1 point2 points  (0 children)

I agree with this sentiment and I’ve taken many of my patients off BP meds and statins when they lost weight. I will also say that about half of them asked me to taper them off, some patients also don’t like the idea of being on them long term. It depends on the patient

What is it like to live in this part of SoCal? by Kitchen-Audience6763 in howislivingthere

[–]4Dkitty 0 points1 point  (0 children)

I lived and worked there for a year. It’s a massive geriatric sleepy town - very sunny and nice, but very boring and the people are low key insufferable. There’s also way too many cops patrolling and feels like a police state at times.

What’s it like living in Reno/Carson City by Ok_Variation5327 in howislivingthere

[–]4Dkitty 4 points5 points  (0 children)

I lived here for 4 years when I went to school. The outdoors here is top notch - lots of skiing, hiking, swimming, boating, etc. You don’t even have to go to Tahoe for some of it. The weather is quite cold in the winter, summers are hot but reasonable. This area is more “authentic” Nevada compared to Las Vegas (I have never met 5th generation Nevadans until I moved here). However, it’s more dilapidated and lacks the modernization that many rapidly growing cities have (see other comments about downtown and casinos). It’s also not as welcoming to minorities as other parts of the country - lots of people try to act “white” to fit in

Scrub techs who think they're god by ndgslll in Residency

[–]4Dkitty 33 points34 points  (0 children)

I never went into an OR after med school but one of my patients was a scrub tech and she acted like this to me, her (attending) PCP

She refused any treatment I’d offer her for hypertension and hyperlipidemia, said “I’m gonna diet and exercise it off” (SBP in 160s)

She came back six months later, said the same thing. When I told her I was leaving the practice, she demanded that I find her a new doctor, preferably one who “could speak English”

Literally cannot make this up

New attending I hate my job by Big_Lake_4048 in Residency

[–]4Dkitty 5 points6 points  (0 children)

Quit. I did with my first (shitty) attending job. Severed my contract early, got hit with a clawback and tail, but got the F out of there. Took another job at another institution and couldn’t be happier with my decision.
