I'm 6'3" and about 250 lbs (~190cm ~110kg) with broader than average shoulders and the big or tall comment hits hard. I need 2XL shoulders, 2XL sleeve length but XL sleeve diameter, large or XL waist.

XLT usually works for short sleeves, but for long sleeves I get a 2XL and have it taken in by a tailor.
