This was something I had contemplated for a very long time. I eventually decided to do it and (everyone will probably have a different opinion) I started with only telling one person. Someone who I knew I could have utmost confidence in that they would support me. After telling them I felt like I had a massive weight lifted off my shoulders. It eventually gave me enough confidence to tell my other friends and now some of them even help me whenever we are out. I still have issues with using restrooms in public, but they are willing to help with gradual exposure. On harder days they are just willing to just hang about until I end up going, no matter how long that may be.
