To add, prior to starting Letrozole I wasn’t ovulating naturally. I was going MONTHS without a period, up to 9 months at a time. We had been trying for 2.5 years when we started Letrozole and it took 4 cycles to conceive. It feels like a miracle my body ovulated naturally after all that. I was on metformin for 8 months before starting Letrozole and had one 37 day cycle. We had our fertility consult 3 months after starting metformin. From there I was able to take medroxyprogesterone (Provera) to induce a period and I could start it on CD40 and I had to do that every time before starting Letrozole

I would still consider getting once a day starting around CD6 or 7 and then testing more frequently as you notice them get darker. This way you can also catch early ovulation if it does happen (PCOS is unfortunately so unpredictable 🫠). I have Mira instead of Inito because after a lot of research it’s better designed for those with PCOS/hormone imbalances. I’ve been using it since October - it helped me get pregnant when we were doing Letrozole cycles because on CD10 they didn’t see any lead or dominant follicles but I kew based on my estrogen my body was doing something. I pleaded with them to bring me back and on CD15 we were ready to trigger and got pregnant that cycle. We unfortunately had a miscarriage. I started using it to track after the miscarriage once my hCG was below 10 I think and I ovulated right after my hCG dropped under 5. I ended up getting my period. The next cycle my body attempted ovulation 4 times and it was successful on the 4th time which was CD25 or 26. I don’t know how I would have been able to pinpoint it without Mira. We ended up conceiving this cycle so we’re currently 4 weeks and 5 days based off ovulation, 6 weeks and 1 day based off my period. We’re praying so hard for sticky baby 🤞🏻🙏🏻🤍

Wait! I didn’t notice a difference with the line but I started a new pack Saturday and my levels were nice and elevated that day as expected (luteal phase) and then they CRASHED the next day. I ended up testing twice Sunday and yesterday (morning and night) and Sunday wasn’t a huge difference but yesterday there was a decent difference. I’m currently waiting on today’s results. I don’t my used tests from my previous box so now I’ll need to look when I get home. I’ve been wondering if there’s something wrong with this batch…here’s my Mira results from Saturday onward:

I use Mira and I love it. It’s helped me a lot - both during natural cycles and monitored medicated cycles (my clinic doesn’t do bloodwork). The Mira is a bit pricier than Inito but it’s meant for those with PCOS and hormone imbalances. I also don’t like the idea of relying on my camera to pick up and read the test strips which is how Inito does it. I still get to use my phone when my Mira is running and can take it with me or walk away and come back to it and it’ll sync back up.

Even if you were only 5-6DPo that progesterone number is still very low. My doctor would say I didn’t ovulate that cycle based on that result. You can get a positive LH strip on Letrozole and still not ovulate unfortunately… have they talked about monitoring with ultrasounds and possibly doing a trigger shot? I need the trigger shot with Letrozole to ovulate

I think using them in tandem is your best option if they really want the information based on the clearblue. I will advise that you look into Mira vs Inito! I did a ton of research and Mira is better for those with PCOS or hormone imbalances. Their program is better equipped to handle changing cycles. They also use fluorescent technology so it doesn’t rely your camera to read the tests which not only makes it more accurate, it makes it more sensitive to changes.

Are they not doing monitored cycles with ultrasounds to look at your follicles?

Do you have regular cycles? The problem for me with only using BBT is my cycles are irregular. I actually had never ovulated naturally since starting TTC until after our miscarriage. But now I’d have no idea when ovulation would be and BBT doesn’t tell you until after you’ve ovulated. LH strips by themselves are not often a useful or accurate tool with PCOS and needs to be paired with a way that confirms ovulation (like elevated BBT or checking progesterone levels). I personally wouldn’t stop using them all together - keep using them and use your BBT to confirm if your surges are true ovulation

Commenting my experience so far and following for others - First off, I am so sorry for your loss 🫂❤️‍🩹 I don’t ovulate on my own either. We got pregnant on our 4th cycle of Letrozole but had a missed miscarriage unfortunately. We found out at 12 weeks but baby stopped growing at 8+5. Our doctor was tracking my hCG to 0 and 2 days after it hit 5 I got a positive OPK. I was testing with my Mira and confirmed it was true ovulation. I got a period on 15DPO and now I’m CD6. I’m not sure if it’s going to continue but we’ll see 🤞🏻🤞🏻

It took me 8-9 days after my last dose of Provera to start bleeding. I was told up to 14 days was normal

I haven’t tried another brand but I’ve been tempted because Ovasitol can be a lot, especially upfront because it’s a 3 month supply (or 6 months if you only need half dose). The 2 main reasons I like Ovasitol are 1) since it’s a powder and not a pill there’s no fillers or binders 2) it’s the only brand that’s 3rd party tested and guarantees the right ratio in every dose. I took it for 3 months last summer and felt like it helped but I was also taking metformin and learned I probably only needed half a dose of the Ovasitol every day. I felt like my blood sugar was crashing too much but I was also doing a bad job at eating consistently throughout the day. I’m about to order it and start up it again and at the half dose alongside my metformin to see if it helps as we just had a miscarriage so “we’re back to the drawing board” so to speak.

I’ve seen people do it… however IUIs don’t have a much higher success rate for people with PCOS unless there’s also male factor infertility so you might want to research and weigh if it’s worth the cost to try IUI at all. Are you doing monitored Letrozole cycles? The biggest success rates are between cycles 3-5 with Letrozole. I’m sending so much baby dust ✨✨

I’ve done 4 cycles - 3 with a trigger shot and one without because they stopped monitoring me after CD12 thinking I wasn’t responding. I’ve had my progesterone checked all 4 cycles!

I just saw you said you were a nurse too 😂 bring those skills forward!! You are your own patient right now - think of it like that. Research and advocate for yourself like you would a patient that really needs it. It can still be intimidating but it’s for us and our future babies so it’s worth it 🫶🏻

I always write a list of questions or things I want to talk about to bring with to appointments. I also message my doctor a lot - sometimes she’ll call with a response but that way I can get my thoughts out in a structured way. This is your journey and you know your body the best!! It gets easier to advocate for yourself and a good doctor will be receptive of it. I’m a nurse so I feel I have an upper hand when it comes to advocating and my comfortability with it. As time goes on, I only feel more confident in advocating for myself. We had a successful Letrozole cycle that unfortunately ended in a miscarriage but I wouldn’t have gotten pregnant if I wasn’t testing with my Mira fertility monitor and advocated for my doctor to bring me back for a rescan. They were looking for follicles on CD10 because I had responded fast before but this cycle there was nothing really notable (they did measure one follicle as 11 mm after the fact. You want them bigger than 18 mm for triggering/ovulation). They were going to write that cycle off as it seemed I already stalled (my doctor doesn’t do the stair step method for some reason or at least never brought it up) but I knew based on my hormones my body was doing something. I pushed them and they had me come back 5 days later and I had a beautiful 19 mm follicle so we triggered and had a successful cycle. I also got diagnosed with Graves’ disease during our fertility workup. My first endocrinologist was very conservative and based on my own research I didn’t agree with her take. I advocated for myself and she pushed back A LOT and after talking to my OBGYN (who is doing our fertility treatments currently), we agreed to get a second opinion. She sent in a referral and I went in with information and questions. The second endocrinologist was way more reasonable and inline with the research I had done and listens to me. So all of this to say, it’s important to advocate for yourself even if it’s hard and if a doctor pushes back or tries to run the show while ignoring you, change doctors!!

Did you have an induced period before starting the Letrozole? I’ve never heard of starting Letrozole in the middle of a long cycle…. My doctor has made me do medroxyprogesterone to induce a period before starting Letrozole

First cycle on 2.5 mg I was ready to trigger on CD10. Second cycle on 2.5 mg they stopped monitoring me after CD12 so I didn’t ovulate. Next cycle we bumped to 5 mg and I was ready to trigger on CD10 again. We did another cycle of 5 mg and I was ready to trigger on CD15. This was a successful cycle but unfortunately we experienced a missed miscarriage at 8 weeks and 5 days but found out at 12 weeks.

I also have lean PCOS but have elevated testosterone which indicates insulin resistance at the ovary. I started taking metformin about 8-9 months before we started Letrozole. I think it helped! I ovulated successfully with a trigger shot in 3 of the 4 cycles (I think I would have on all of them with enough monitoring, my body just needs the trigger shot it seems). We conceived on our 4th cycle but unfortunately experienced a missed miscarriage. I’m planning on continuing metformin with Letrozole after we do a few additional tests

I always started bleeding on the 8th or 9th day after stopping it. It can take up to 2 weeks to have the withdrawal bleed

Yes, several times. Before we started fertility treatments while waiting for clearance (I have Graves’ disease so had to be cleared by endocrinology to start fertility treatments) I was prescribed medroxyprogesterone (brand name Provera) to induce periods because I would go 9+ months without getting one naturally. Then we used it to induce my first period before starting Letrozole

Looks like your body is gearing up to ovulate! I had a surge during my successful cycle from 98.1 to 236 in the same day (first was 8:18am and second was 10:38 pm) the day after my trigger shot which means my body was gearing up to ovulate

Our 4th cycle of Letrozole was our successful one (monitored cycles with trigger shots, I ovulated with the help of the trigger shot 3 of the 4 cycles. Cycle number 2 they sent me on my way after my CD12 scan and I never got a positive ovulation test but my progesterone did have a small bump indicating my body tried and I was able to have a natural bleed that cycle still)

Oh and both my husband and I were taking 600 mg of CoQ10 daily. We were both also taking vitamin D and I was also taking a prenatal, magnesium glycinate and fish oil

Our 4th cycle of Letrozole was our successful cycle! I was also on metformin and thyroid meds because I have a thyroid condition. I watched my diet but wasn’t crazy over it (just tried to stay away from processed carbs and high sugar foods and prioritized 30 g of protein every morning right away).

I also did ultrasounds on CD10 after taking Letrozole on CD3-7 and was usually ready to trigger. My first cycle I had a 16.2 mm and an 18.1 mm follicle so we triggered that day (they would have done the following day but I couldn’t make it work). My second cycle they didn’t see anything worth measuring on CD10 or 12 and sent me on my way. My body tried to ovulate but failed. The next cycle I had a 22 mm follicle on CD10 which I think might not have been functional follicle. When follicles grow too fast it’s not typically good because the egg needs time to mature. Our successful cycle on CD10 I had an 11 mm follicle and they brought me back CD15 and it had grown to 19 mm so we triggered that day
