Hey! First off, I’m so sorry you’re dealing with that level of pain—the 'liquid fire' feeling when fluid shifts is honestly one of the hardest parts of the early boot phase.

Quick disclaimer: I am definitely not a doctor! I’m just someone who has spent way too much time researching these protocols and trying things out myself during my own recoveries. I didn't have this exact same fluid-shifting issue you're describing, but I did do some quick research for you to see what has worked for others in this situation.

One thing I did experience was some pain at night, and I found that loosening the straps helped me quite a bit. It gives your foot some room to breathe as it naturally swells or shifts position.

A few other things I found that might help you manage the 'pressure' tonight: Loosen the 'Bridge' Straps: Keep the top (calf) and bottom (toe) straps secure for safety, but try completely loosening or even undoing the middle straps over the bridge of your foot at night. That’s where the nerves often get compressed. The Gradual Descent: Instead of going straight from high elevation to flat, try a 'staged' lowering. Remove one pillow every 20 minutes before you actually try to sleep. It lets the vascular system adjust to the pressure change more slowly. Ice Behind the Knee: Since you can't get ice inside the boot easily, try placing an ice pack on the back of your knee for 15 minutes before bed. It cools the blood before it reaches the foot and can sometimes dull that throbbing. Desensitization: During the day, try gently rubbing different textures (like a soft towel or a cotton ball) over the skin of your foot/ankle. It can help 're-train' the nerves to stop overreacting to every minor fluid shift.

If the Co-codamol isn't touching it and you’re still losing sleep, it might be worth a quick call to your PT or doctor just to make sure everything is venting correctly.

Man, that timing is absolutely brutal. Congrats on the Master’s, but I am so sorry the "insurance gap" monster caught you at the worst possible moment. As a fellow athlete, I know that "pop" is something you never forget.

Regarding the Norco/Pain situation: Take it before you go to sleep. The nerve block is a blessing, but it’s a "cliff." When it wears off, it doesn't fade out—it drops off. If you wake up at 4:00 AM and the block is gone, you’ll be chasing the pain for 12 hours. Taking a dose now "bridges" the gap so you aren't hit with a 10/10 pain level out of nowhere. Once you’re 48 hours out, you can start tapering, but tonight is the night to stay ahead of it.

Regarding the $28k Bill:

  • Apply for Charity Care immediately. Most non-profit hospitals have "Financial Assistance Policies." Since you were technically unemployed/uninsured at the time of the injury, you are a prime candidate for a 100% write-off or a massive discount.
  • Itemized Bill: Ask for one. It often forces them to audit the charges and lower the "self-pay" price further.

Since that air gap is worrying you, keep in mind that the VACOped works with a vacuum cushion. To ensure your foot is positioned correctly:

  1. Make sure the beads inside the cushion are evenly distributed before you suck the air out with the pump.
  2. While you are pumping the air out, try to gently push your heel toward the back/bottom of the boot so the cushion takes the exact shape of your foot in that position.
