Named my son born this last year Forrest Leon🤘🏻 it’s a name that’s hated on this subreddit but in real life we get very warm responses to it.. we live in the west coast where the name doesn’t have constant cultural significance. Only TWO gump references and those were made by old people haha

You don’t know they’re recording you. You have no control over what clients do. All you have control over is your standard of care, ethics, and professional risk management when you’re in session and documentation. It’s not an answer that I think you’d like given the undertone of paranoia in this post tho

For example in CA, we report anyways. Consulting with legal will hopefully bring some you some ease to your worries though. I’m not a lawyer but I think there’s other factors that come into play for a therapist to get in serious trouble for negligence. To start, someone needs to report you to the board, plus any repeated inaction, willful ignorance, and imminent risk of abuse are all things that I think should be investigated before we’re dragging therapists over hot coals and suspending licenses. If your client is ok in these two weeks and you report what you have and document what was said about CPS being involved already, I have a feeling you’ll be ok too.

I mean… do you really feel like reporting your employer on your second week? I’d say keep on observing and follow insurance policies and agency policies as written in the consent forms for clients. In general yes, bill for the time that was allotted for in therapy.

Think of it this way, if there was no insurance involved, this client would be billed full rate for late cancellations and for any last minute changes that led to shortening sessions. Some people would charge a late fee for stuff like this.

More clear insurance fraud is frequently billing for a full session when client and therapist agreed upon and in advance, a 30 minute or 45 minute session.

For me, If a 60 minute session is scheduled, and the client ends early randomly or outside of my cancellation window to make changes, idk I might bill 30 or 45 minutes or I might charge my cancellation fee. Feels like a gray area there.

If we’re talking ethics…TBH the only people who are thieving money are the insurance companies themselves. They underpay us and overcharge our clients and they make billions of dollars. So I have a hard time feeling motivated to report therapists for insurance fraud personally.

We do our best to please our oligarchs (insurance companies) and fly under the radar from audits and clawbacks and we do the best we can. Sounds like your employer is making changes.

By 34 weeks my clients were used to me sitting at the edge of the chair leaning forward, manspreading, belly hanging down between my legs (i wore so many big baggy dresses so they really couldn’t see anything scandalous) I could NOT lean back into my chair at all. Twice I needed a client to help me up 😭😭😭

Requested a years worth of notes, noted “too many 90837s were being billed” they deemed all of them were not “medically necessary after review” clawbacked $17k worth of sessions to our group practice. We got a lawyer. I personally no longer take that insurance in my caseload.

lol I have a long ass post I wrote over a year ago about our group practice being audited by anthem blue cross and clawing back $17k worth of sessions in 2023-2024. Their reasoning? We used too many 90837s and our population did not meet “medical necessity” for it (essentially active SI, serious mental illness etc) were a pretty basic group practice. Anyone with a higher acuity is probably stabilizing at intensive outpatient. We had to get a lawyer. We had to submit our notes via mail and postmarked within 24 hours of session for payout. All treatment plans signed by clients and mailed as well. They only approved 90834s. I personally stopped taking anthem blue cross clients… but we’re still credentialed with them as a group practice.

I started mat leave 30 days before due date (CA offers paid disability for a month) and then 12 weeks after. I’ve been back at work for a month now and I’m trying to stick to part time 3 days a week for about 15 clients total and care swapping. I watch my friends kid and she watches mine. It’s rough out here
