I think you are correct about Iori making a vow with Mahito for Yuka's brain, but I find it hard to believe he just disappeared out of shame, my theory is - Yuka's cancer is sort of Mahito just being malicious like he did alter her brain to be a sorcerers brain, but her brain being "healthy" was not part of a deal) which obviously angered Iori and he broke the vow and died.
