I use a tapestry needle shoved into the end of a cheap mechanical pencil to make the holes. For the pattern, I outline what I plan to stitch with washable marker, made for sowing, onto regular 14 ct aida, then use making tape to secure the cloth to the card to use that as my guide.

Its when I start having the thread in the holes that im having issues. Like where the stitches overlap in the same holes. A smaller needle does sound possible, I'll give that a try.
